The components of reassurance for no-problem diagnoses
Deflecting Before Prescribing
In this study, we examine opioid-related treatment negotiations during primary care visits for chronic pain. We video-recorded 86 visits involving established patients on chronic opioids seeing primary care physicians; participants completed post-visit assessments. We used conversation analysis to identify patient requests (direct and indirect) for better pain control, create a taxonomy of eight "deflection" practices physicians used to respond b…
Communicative Practices Clinicians Use to Correct Patient Misconceptions in Primary Care Visits
To investigate how clinicians correct patient misconceptions, we analyzed 23 video recordings of primary care visits. Analysis focused on operationalizing, identifying, and characterizing clinician corrections, integrating two inductive approaches: microanalysis of clinical interaction and conversation analysis. According to our definition, patient misconception-clinician correction episodes met three essential criteria: (1) the clinician refuted…
Expanding the Scope of the Visit
This study examines patient-initiated additional concern presentations in general surgery visits to assess which factors contribute to them getting “helped.” Concern presentations were analyzed for a variety of design features (e.g., social action (inquiry vs. informing), when patients initiate during a visit, how patients fit topic to previous talk or activity) and contextual factors (e.g., concern falls inside or outside surgeon’s domain of exp…
Making Conversation Analysis Accessible
The quality of healthcare communication can impact both experiences and outcomes. We highlight aspects of communication that can be systematically examined using Conversation Analysis (CA) and provide guidance about how researchers can incorporate CA into healthcare studies. CA is a qualitative method for studying naturally occurring communication by analyzing recurrent, systematic practices of verbal and nonverbal behavior. CA involves examining…
On the recognitionality of references to time in social interaction
This article explores the recognitionality of references to time as a participants’ category and resource in social interaction. In short, we ask: How does referring to time in more vs. less recognitional ways contribute to the formation and ascription of action in context, and how can analysts of social interaction approach this dimension of reference in ways that remain grounded in the details of participant joint-conduct? After considering som…
When and how do surgeons initiate noticings of additional concerns
Authority and camaraderie
This analysis focuses on the institutional talk of sea-kayak guides and their clients in order to understand how guides negotiate the interactional balance of giving orders to maintain a safe and timely excursion while facilitating a fun and recreational experience. Using a mixed-method analysis including Conversation Analysis, ethnography, and statistics, this study examines 576 instances of directives found in video recordings of twenty-five Al…
Time Reference in the Service of Social Action
The present study investigates the ways that members of society refer to time. Concrete methods for communicating about points in time and locating events in relation to them make relevant and thereby ground abstract time-reckoning in the lives of interactants. Through a taxonomy of references to time—termed absolute and event-relative, each with subcategories—we describe the intrinsic affordances that different designs provide coparticipants eng…
