  • Multilevel Factors Influencing Linkage to HIV Care in a High HIV Prevalence District in South Africa

Open Access•Edward Nicol, Wisdom Basera et al.•ARTICLE•AIDS and Behavior•2026

Timely linkage to HIV care is essential for achieving South Africa’s treatment goals, yet significant gaps persist in the early stages of the HIV care continuum. This study examined multilevel factors influencing linkage to care in a high HIV-prevalence rural district. A prospective convergent mixed-methods cohort study was conducted over 21 months among 1,194 adults newly diagnosed with HIV across 18 facilities in KwaZulu-Natal. Quantitative ana…

Open Access•Nigel Garrett, Tarylee Reddy et al.•ARTICLE•PLOS Global Public Health•2024

Limited studies have been conducted on the safety and effectiveness of heterologous COVID-19 vaccine boosting in lower income settings, especially those with high-HIV prevalence., The Sisonke Heterologous mRNA-1273 boost after prime with Ad26.COV2.S (SHERPA) trial evaluated a mRNA-1273 boost after Ad26.COV2.S priming in South Africa. SHERPA was a single-arm, open-label, phase 3 study nested in the Sisonke implementation trial of 500000 healthcare…

  • Can verbal autopsies be used on a national scale? Key findings and lessons from South Africa's national cause-of-death validation study

Open Access•Monique Maqungo, Nadine Nannan et al.•ARTICLE•Global Health Action•2024

BACKGROUND: Verbal autopsy (VA), though imperfect, serves as a vital tool to determine cause-of-death, particularly for out-of-facility deaths, but challenges persist in integrating VA into Civil Registration and Vital Statistics systems. OBJECTIVE: To describe the challenges and successes of collecting a national sample of verbal autopsy interviews in South Africa to obtain the cause of death profile in 2017/18. METHODS: We recruited next of kin…

  • Injury mortality in South Africa

Open Access•Megan Prinsloo, Shibe Mhlongo et al.•ARTICLE•Global Health Action•2024

BACKGROUND: Injuries, often preventable, prompted urgent action within the United Nations' 2030 Agenda for Sustainable Development Goals (SDGs) to improve global health. South Africa (SA) has high rates of injury mortality, but accurate reporting of official national data is hindered by death misclassification. OBJECTIVE: Two nationally representative surveys for 2009 and 2017 are utilised to assess SA's progress towards SDG targets for violence …

  • Alcohol, Violence, and Injury-Induced Mortality

Open Access•Kai Barron, Charles D H Parry et al.•ARTICLE•The Review of Economics and…•2024

This paper evaluates the impact of a sudden and unexpected nationwide alcohol sales ban in South Africa. We find that this policy causally reduced injury-induced mortality in the country by at least 14%. We argue that this estimate constitutes a lower bound on the true impact of alcohol on injury-induced mortality. We also document a sharp drop in violent crimes, indicating a tight link between alcohol and aggressive behavior in society. Our resu…

Open Access•Pam Groenewald, Jason R Thomas et al.•ARTICLE•Global Health Action•2023

BACKGROUND: The South African national cause of death validation (NCODV 2017/18) project collected a national sample of verbal autopsies (VA) with cause of death (COD) assignment by physician-coded VA (PCVA) and computer-coded VA (CCVA). OBJECTIVE: The performance of three CCVA algorithms (InterVA-5, InSilicoVA and Tariff 2.0) in assigning a COD was compared with PCVA (reference standard). METHODS: Seven performance metrics assessed individual an…

  • Enhancing linkage to HIV care in the “Universal Test and Treat” era

Open Access•Edward Nicol, Ngcwalisa Jama et al.•ARTICLE•BMC Public Health•2023

Ending AIDS by 2030 would depend on how successful health systems are in linking people living with HIV (PLHIV) into care. The World Health Organization recommended the 'Universal Test and Treat' (UTT) strategy - initiating all individuals testing positive on antiretroviral therapy (ART) irrespective of their CD4 count and clinical staging. This study aimed to explore the enablers and barriers to linkage to HIV care among adults with a new HIV di…

  • Linkage to HIV Care and Early Retention in Care Rates in the Universal Test-and-Treat Era

Open Access•Edward Nicol, Wisdom Basera et al.•ARTICLE•AIDS and Behavior•2022

HIV linkage, and retention are key weaknesses in South Africa's national antiretroviral therapy (ART) program, with the greatest loss of patients in the HIV treatment pathway occurring before ART initiation. This study investigated linkage-to and early-retention-in-care (LTRIC) rates among adults newly diagnosed with HIV in a high-HIV prevalent rural district. We conducted an observational prospective cohort study to investigate LTRIC rates for a…

  • Can non-fatal burden estimates from the Global Burden of Disease study be used locally? An investigation using models of stroke and diabetes for South Africa

Open Access•Victoria Pillay‐van Wyk, Rifqah Abeeda Roomaney et al.•ARTICLE•Global Health Action•2021

Background: The Global Burden of Disease (GBD) approach estimates disease burden by combining fatal (years of life lost) and non-fatal burden prevalence-based years of life lived with disability (PYLDs) estimates. Although South Africa has data to estimate mortality, prevalence data to estimate non-fatal burden are sparse. PYLD estimates from the GBD study for South Africa can potentially be used. However, there is a divergence in mortality estim…

  • Is the routine health information system ready to support the planned national health insurance scheme in South Africa

Open Access•Edward Nicol, Lyn A Hanmer et al.•ARTICLE•Health Policy and Planning•2021

Implementation of a National Health Insurance (NHI) in South Africa requires a reliable, standardized health information system that supports Diagnosis-Related Groupers for reimbursements and resource management. We assessed the quality of inpatient health records, the availability of standard discharge summaries and coded clinical data and the congruence between inpatient health records and discharge summaries in public-sector hospitals to suppo…

  • Global surveillance of trends in cancer survival 2000–14 (Concord-3)

Open Access•Claudia Allemani, Tomohiro Matsuda et al.•ARTICLE•The Lancet•2018

BACKGROUND: In 2015, the second cycle of the CONCORD programme established global surveillance of cancer survival as a metric of the effectiveness of health systems and to inform global policy on cancer control. CONCORD-3 updates the worldwide surveillance of cancer survival to 2014. METHODS: CONCORD-3 includes individual records for 37·5 million patients diagnosed with cancer during the 15-year period 2000-14. Data were provided by 322 populatio…

  • Mortality trends and differentials in South Africa from 1997 to 2012

    Open Access•Victoria Pillay-Van Wyk, Victoria Pillay‐van Wyk et al.•ARTICLE•The Lancet Global Health•2016

BACKGROUND: The poor health of South Africans is known to be associated with a quadruple disease burden. In the second National Burden of Disease (NBD) study, we aimed to analyse cause of death data for 1997-2012 and develop national, population group, and provincial estimates of the levels and causes of mortality. METHOD: We used underlying cause of death data from death notifications for 1997-2012 obtained from Statistics South Africa. These da…

  • Disability weights from a household survey in a low socio-economic setting

Open Access•I Neethling, Jennifer Jelsma et al.•ARTICLE•Global Health Action•2016

BACKGROUND: The global burden of disease (GBD) 2010 study used a universal set of disability weights to estimate disability adjusted life years (DALYs) by country. However, it is not clear whether these weights can be applied universally in calculating DALYs to inform local decision-making. This study derived disability weights for a resource-constrained community in Cape Town, South Africa, and interrogated whether the GBD 2010 disability weight…

  • Urban–rural and gender differences in tobacco and alcohol use, diet and physical activity among young black South Africans between 1998 and 2003

Open Access•Nasheeta Peer, Debbie Bradshaw et al.•ARTICLE•Global Health Action•2013

BACKGROUND: Non-communicable chronic diseases (NCDs) have increased in South Africa over the past 15 years. While these usually manifest during mid-to-late adulthood, the development of modifiable risk factors that contribute to NCDs are usually adopted early in life. OBJECTIVE: To describe the urban-rural and gender patterns of NCD risk factors in black adolescents and young adults (15- to 24-year-olds) from two South African Demographic and Hea…

  • Health in South Africa

Open Access•Bongani M Mayosi, Joy Elizabeth Lawn et al.•ARTICLE•The Lancet•2012

  • Characteristics, availability and uses of vital registration and other mortality data sources in post-democracy South Africa

Open Access•Jané Joubert, Chalapati Rao et al.•ARTICLE•Global Health Action•2012

The value of good-quality mortality data for public health is widely acknowledged. While effective civil registration systems remains the 'gold standard' source for continuous mortality measurement, less than 25% of deaths are registered in most African countries. Alternative data collection systems can provide mortality data to complement those from civil registration, given an understanding of data source characteristics and data quality. We ai…
