Understanding the role of family social support in the medication adherence to hypertension treatment in Cambodia
The role of social support from family members was very positive in enhancing medication adherence among patients with HTN in Cambodia. Potential strategic interventions should include focus on establishing an intervention involving family members to provide support for HTN treatment
Exploring different health care providers ́ perceptions on the management of diarrhoea in cholera hotspots in the Democratic Republic of Congo
Global cholera guidelines support wider healthcare system strengthening interventions, alongside vertical outbreak responses, to end cholera. Well-trained healthcare providers are essential for a resilient health system and can create synergies with childhood diarrhoea, which has higher mortality. We explored how the main provider groups for diarrhoea in cholera hotspots interact, decide on treatment, and reflect on possible limiting factors and …
Hardship financing, productivity loss, and the economic cost of illness and injury in Cambodia
Such losses can be mitigated with policy measures such as linking a catastrophic health coverage mechanism to the Health Equity Funds, capping interest rates on health-related loans, and using loan guarantees to incentivize microfinance institutions and banks to refinance health-related, high-interest loans from money lenders. These measures could strengthen social health protection by enhancing financial risk protection, mitigating vulnerability…
Can social accountability improve access to free public health care for the poor? Analysis of three Health Equity Fund configurations in Cambodia, 2015–17
Within the context of universal health coverage, community participation has been identified as instrumental to facilitate access to health services. Social accountability whereby citizens hold providers and policymakers accountable is one popular approach. This article describes one example, that of Community-Managed Health Equity Funds (CMHEFs), as an approach to community engagement in Cambodia to improve poor people’s use of their entitlement…
Does public service motivation matter in Moroccan public hospitals? A multiple embedded case study
This study highlights the presence of PSM as a driver of motivation among health workers in four Moroccon hospitals, and the prominence of intrinsic motivation and compassion in the motivation of frontline health workers. Religious beliefs were found to shape the expression of PSM in Morocco
The uncertain future of lay counsellors
Between 2006 and 2011, when antiretroviral therapy (ART) was scaled up in a context of severe human resources shortages, transferring responsibility for elements in human immunodeficiency virus (HIV) care from conventional health workers to lay counsellors (LCs) contributed to increased uptake of HIV services in Lesotho. HIV tests rose from 79 394 in 2006 to 274 240 in 2011 and, in that same period, the number of people on ART increased from 17 3…
The role of ‘hidden’ community volunteers in community-based health service delivery platforms
Community-based research on child survival in sub-Saharan Africa has focussed on the increased provision of curative health services by a formalised cadre of lay community health workers (CHWs), but we have identified a particular configuration, that deserves closer scrutiny. We identified a two-tiered CHW system, with the first tier being the lessor known or 'hidden' community/village level volunteers and the second tier being formal, paid CHWs,…
From conceptual pluralism to practical agreement on policy
BACKGROUND: As the human cost of the global economic crisis becomes apparent the ongoing discussions surrounding the post-2015 global development framework continue at a frenzied pace. Given the scale and scope of increased globalization moving forward in a post-Millennium Development Goals era, to protect and realize health equity for all people, has never been more challenging or more important. The unprecedented nature of global interdependenc…
Adapting a community-based ART delivery model to the patients’ needs
Over the past four years, the modifications in the CAG-model contributed to a patient empowerment and better treatment outcomes. One of the main outstanding questions is how this model will evolve in the future. Close monitoring is essential to ensure quality of care and to maintain the core objective of the CAG-model 'facilitating access to ART care' in a cost and time saving manner
Health systems frameworks in their political context
Health systems theories and frameworks frame health, health systems and policies in particular political and public health paradigms. While there is a clear trend to try to understand the complexity of and dynamic relationships between elements of health systems, there is also a demand to provide frameworks that distinguish between health system interventions, and that allow mapping with a view of analysing their returns. The choice for a particu…

Scaling-up antiretroviral treatment in Southern African countries with human resource shortage
Health System Strengthening and Scaling Up Antiretroviral Therapy
In this issue Schneider and colleagues point to some of the health system challenges for scaling up antiretroviral (ARV) therapy to the majority of the people living with HIV and AIDS (PLWHAs) who will need it over the coming years. They assume that in low-income countries financial resources and supplies of drugs do not constitute anymore the most important obstacles to widespread ARV use. Indeed many of the countries most affected by HIV now ha…
Out‐of‐pocket health expenditure and debt in poor households
Objectives To document how out‐of‐pocket health expenditure can lead to debt in a poor rural area in Cambodia. Methods After a dengue epidemic, 72 households with a dengue patient were interviewed to document health‐seeking behaviour, out‐of‐pocket expenditure, and how they financed such expenditure. One year later, a follow‐up visit investigated how the 26 households with an initial debt had coped with it. Results The amount of out‐of‐pocket hea…
