Treatment as Harm Reduction, Defunding as Harm Maximization
Despite numerous research studies demonstrating the efficacy of methadone maintenance treatment (MMT) in general and the value of retention in particular, the increasing defunding of this modality has compromised its potential. From 1990 to 1995 the lead author conducted a longitudinal research project to determine the impact of the cost of treatment on 233 San Francisco Bay Area study participants seeking, enrolled in, or defunded from MMT. Violence
We investigate the relationships between violence, drug use and methadone maintenance treatment (MMT) for women injection drug users (IDUs). The data presented here come from a longitudinal study of 233 IDUs both in and out of MMT in the San Francisco Bay Area. Each was interviewed five times over a period of three years, both qualitatively and quantitatively. Using grounded theory principals, we analyze 55 women's accounts of violence. We find t…
Living with the Dirty Secret”
This article focuses on the management of stigma by methadone maintenance patients. It explores (1) the consequences of this persistent stigma, which necessitates the need for secrecy, (2) the adaptations made in order to successfully "pass as normal," and (3) the contexts in which methadone-maintained individuals decide to disclose their stigmatized status. The data reported and analyzed were gathered between 1980 and 1989 during a series of stu…
Researcher as Friend
This article reports on a study that was designed to document the experience of clients who were forced, after having been on a methadone maintenance program for 2 years or more, to begin paying in full for their treatment or to detoxify from the program. The 5-year follow-up study's longitudinal scope invited the authors to learn, and in some cases devise, strategies for establishing long-term rapport and trust with their respondents. The author…
