The Economic burden of opioid-related serious adverse events and undertreatment of acute pain in the United States
Acute pain has become increasingly prevalent in the United States due to an aging population, rising metabolic disease rates, and growth in surgical procedures. Opioids remain the primary treatment for moderate-to-severe acute pain but are associated with serious adverse events, including misuse, dependency, opioid use disorder, and overdose. In contrast, restrictive prescribing may result in undertreatment of acute pain and progression to chroni…
Online Simulation Model to Estimate the Total Costs of Tobacco Product Waste in Large U.S. Cities
Tobacco product waste (TPW) is one of the most ubiquitous forms of litter, accumulating in large amounts on streets, highways, sidewalks, beaches, parks, and other public places, and flowing into storm water drains, waste treatment plants, and solid waste collection facilities. In this paper, we evaluate the direct and indirect costs associated with TPW in the 30 largest U.S. cities. We first developed a conceptual framework for the analysis of d…
Tobacco litter costs and public policy
Tobacco litter abatement costs to cities can be substantial, even when the costs of potential environmental pollution and tourism effects are excluded. One public policy option to address tobacco litter is levying of fees on cigarettes sold. The methodology described here for calculating TPL costs and abatement fees may be useful to state and local authorities who are considering adoption of this policy initiative
Tobacco sales in community pharmacies
This study applied multilevel modeling procedures with data from 678 community pharmacies and 382 residential census tracts in a Midwestern U.S. state to determine if two sets of variables: retail type (e.g., remotely owned, independently owned) and population demographics of the tracts in which outlets were located were associated with retail tobacco availability in community pharmacies. Data were derived from three archival sources: listings of…
Factors Associated with Iowa Rural Hospitals' Decision to Convert to Critical Access Hospital Status
Context: The Balanced Budget Act (BBA) of 1997 allowed some rural hospitals meeting certain requirements to convert to Critical Access Hospitals (CAHs) and changed their Medicare reimbursement from prospective to cost‐based. Some subsequent CAH‐related laws reduced restrictions and increased payments, and the number of CAHs grew rapidly. Purpose: To examine factors related to hospitals' decisions to convert and time to CAH conversion. Methods: Ei…
Converting to Critical Access Status
To improve rural access to care, the Balanced Budget Act of 1997 allowed eligible rural hospitals to convert to critical access hospitals (CAHs), which changed their Medicare payment from a prospective payment system (PPS) to a cost-based system. The objective of this paper is to examine the effects of CAH conversion on rural hospital operating revenues, operating expenses, and operating margins using an eight-year panel of 89 rural hospitals in …
Community organizations and sense of community
The Community Organization Sense of Community Scale (COSOC) is a frequently used or cited measure of the construct in community psychology and other disciplines, despite a lack of confirmation of its underlying 4‐factor framework. Two studies were conducted to test the hypothesized structure of the COSOC, the potential effects of method bias on psychometric properties, and whether a revised measure (COSOC‐R) yielded improved model‐to‐data fit. St…
Effects of Specialty Hospitals on the Financial Performance of General Hospitals, 1997–2004
Hospital specialization has become a controversial topic, culminating in a moratorium issued in 2003 by Congress directing the Centers for Medicare and Medicaid Services to cease payments to new physician-owned specialty hospitals for those Medicare and Medicaid patients referred by physicians with a financial interest in the facility. This paper focuses on one important economic question: does the presence of specialty hospitals in a market affe…
Linking social cohesion and gender to intrapersonal and interactional empowerment
Empowerment is a social-action process through which people gain greater control, efficacy, and social justice. One way to develop empowerment is through active, meaningful participation in community groups and activities. Social cohesion is an emerging construct that expands the notion of community participation to include elements such as shared emotional commitment and reciprocity. This study extended previous research by examining the role of…
What Would Be the Effect of Referral to High‐Volume Hospitals in a Largely Rural State
Context: Volume of certain surgical procedures has been linked to patient outcomes. The Leapfrog Group and others have recommended evidence‐based referral using specific volume thresholds for nonemergent cases. The literature is limited on the effect of such referral on hospitals, especially in rural areas. Purpose: To examine the impact of evidence‐based referral by volume standard for 5 hospital procedures (abdominal aortic artery repair, coron…
