Introducing a Computerized Care‐Pathways System for Older Adults in Home‐Care Settings
Introducing computerized care pathways for older adults living at home may be a promising way to improve the clinical dimension of integrated care. Evidence on how to implement them in various home‐care contexts is, however, sparse. A prospective, comparative multiple case study with nested analysis units was conducted across three home‐care settings. Participants included managers, healthcare professionals, and home‐care clients. We used a varie…
Risk of Frequent Emergency Department Use Among an Ambulatory Care Sensitive Condition Population
BACKGROUND: A small fraction of patients use a disproportionately large amount of emergency department (ED) resources. Identifying these patients, especially those with ambulatory care sensitive conditions (ACSC), would allow health care professionals to enhance their outpatient care. OBJECTIVE: The objectives of the study were to determine predictive factors associated with frequent ED use in a Quebec adult population with ACSCs and to compare s…
Perspectives of Technology Users on the Eve of Innovation Deployment
Background: Over the last 10 years or so, integrated service networks (ISNs) for older people have been implemented throughout the Province of Quebec in Canada. The main instruments used in the ISNs are the Multiclientele Assessment Tool (OEMC; French acronym), including the Functional Autonomy Measurement System (SMAF); the Iso-SMAF profile classification system; and the Individualized service and allocation plan. All of these instruments are in…
Development of a Computerized Integrated-Care-Pathway System to Support People-Centred and Integrated Care
Background: Integrated service networks (ISNs) have gradually been established for older people in the province of Quebec, Canada. Despite many improvements in the organization of our health-care system, some gaps associated with assessing, planning, and delivering health care and social services reflecting the client’s values and preferences have been underlined. To resolve this situation, we have conducted significant research that allowed the …
Comparing the resident populations of private and public long-term care facilities over a 15-year period
In the province of Quebec, Canada, long-term residential care is provided by two types of facility: privately owned facilities in which care is privately financed and delivered and publicly subsidised accredited facilities. There are few comparative data on the residents served by the private and public sectors, and none on whether their respective population has changed over time. Such knowledge would help plan services for older adults who can …
Alternate housing models for older people with disabilities
This paper reports on the study of a subsidy programme that was established in Quebec for alternate housing models (AHMs), which allows private and community organisations to offer housing services within the framework of a partnership with public health-care services. The research objectives were: (a) to compare how facility characteristics and services provided by AHMs and nursing homes (NHs) differ; (b) to examine the personal characteristics …
Measuring Social-Work Activities with Older People
The aim of this pilot study was to obtain accurate and valid data on how much time social workers spend on their different activities in the home-care programme. Data collection required two steps. Beginning with a list of social-work activities was developed and validated. The final list consisted of 75 activities in four main categories: direct activities, indirect activities, non-clinical activities and personal time. The list was programmed i…
Development of integrated care pathways
Once computerized, these ICPs will facilitate the exchange of information as well as the clinical decision-making process with a perspective to adequately matching the needs of an individual person with resources that delay or slow the progression of frailty and disability. Once aggregated, the data will also support managers in organizing teamwork and follow-up for clients
Impact of PRISMA, a Coordination-Type Integrated Service Delivery System for Frail Older People in Quebec (Canada)
The PRISMA model improves the efficacy of the health care system for frail older people
Is there agreement between Canadian older adults and their primary informal caregivers on behaviour towards institutionalisation
We aimed to compare behaviour towards institutionalisation between frail older adults and their informal caregivers, and identify correlates of differential behaviour. In 2004, during the fourth wave of the longitudinal Program of Research to Integrate Services for the Maintenance of Autonomy (PRISMA) study (province of Québec, Canada), 86.7% of eligible dyads, that is 593 participants and their primary informal caregivers, were asked separately …
The effectiveness of the PRISMA integrated service delivery network
This first year must be considered a baseline year, showing the situation without significant implementation of PRISMA ISD systems. Results for the following years will have to be examined with consideration of these baseline results
Home-care programmes for older adults with disabilities in Canada
The need for home care will probably continue to increase over the next decade as one response to innovative health practices designed to help people receive services at home instead of in institutions. However, there are no data for determining whether home-care programmes can meet user needs. The objectives of the present study were to describe the functional autonomy profile of the users of public home-care programmes in Québec, Canada, and to…
