Monitoring the South African population’s salt intake
Objective The present study set out to determine whether morning spot urine samples can be used to monitor Na (and K) intake levels in South Africa, instead of the ‘gold standard’ 24 h urine sample. Design Participants collected one 24 h and one spot urine sample for Na and K analysis, after which estimations using three different formulas (Kawasaki, Tanaka and INTERSALT) were calculated. Setting Between 2013 and 2015, urine samples were collecte…
Socioeconomic predictors and consequences of depression among primary care attenders with non-communicable diseases in the Western Cape, South Africa
The trial is registered with Current Controlled Trials ( ISRCTN20283604 )
Rising alcohol consumption and a high prevalence of problem drinking in black men and women in Cape Town
BACKGROUND: To determine the prevalence and determinants of problematic alcohol use (CAGE ≥2) in 25-74-year-old black population in Cape Town in 2008/2009 and examine the changes in self-reported alcohol consumption between 1990 and 2008/2009 in 25-64-year-olds. METHODS: In 2008/2009, a representative cross-sectional sample, stratified for age and sex, was randomly selected from the same townships sampled in 1990. Socio-demographic characteristic…
Urban–rural and gender differences in tobacco and alcohol use, diet and physical activity among young black South Africans between 1998 and 2003
BACKGROUND: Non-communicable chronic diseases (NCDs) have increased in South Africa over the past 15 years. While these usually manifest during mid-to-late adulthood, the development of modifiable risk factors that contribute to NCDs are usually adopted early in life. OBJECTIVE: To describe the urban-rural and gender patterns of NCD risk factors in black adolescents and young adults (15- to 24-year-olds) from two South African Demographic and Hea…
Implementation of national guidelines, incorporated within structured diabetes and hypertension records at primary level care in Cape Town, South Africa
BACKGROUND AND OBJECTIVES: Many clinical management guidelines for chronic diseases have been published, but they have not been put into practice by busy clinicians at primary care levels. This study evaluates the implementation of national guidelines incorporated within a structured diabetes and hypertension clinical record (SR) in Cape Town in a randomised controlled trial (RCT). METHODS: Eighteen public sector community health centres (CHC) we…
Pregnant women's responses to a tailored smoking cessation intervention
BACKGROUND: Cognitive behavioral interventions consisting of brief counseling and the provision of self-help material designed for pregnancy have been documented as effective smoking cessation interventions for pregnant women. However, there is a need to understand how such interventions are perceived by the targeted group. AIM: To understand the cognitive, emotional, and behavioral responses of pregnant women to a clinic-based smoking cessation …
Development and validation of a short questionnaire to assess sodium intake
Objectives To develop and validate a short food-frequency questionnaire to assess habitual dietary salt intake in South Africans and to allow classification of individuals according to intakes above or below the maximum recommended intake of 6 g salt day −1 . Design Cross-sectional validation study in 324 conveniently sampled men and women. Methods Repeated 24-hour urinary Na values and 24-hour dietary recalls were obtained on three occasions. Fo…
A food-based dietary strategy lowers blood pressure in a low socio-economic setting
Objective To assess the impact of a food-based intervention on blood pressure (BP) in free-living South African men and women aged 50–75 years, with drug-treated mild-to-moderate hypertension. Methods A double-blind controlled trial was undertaken in eighty drug-treated mild-to-moderate hypertensive subjects randomised to an intervention ( n 40) or control ( n 40) arm. The intervention was 8-week provision of six food items with a modified cation…
Obesity in South Africa
Objectives: To ascertain the anthropometric profile and determinants of obesity in South Africans who participated in the Demographic and Health Survey in 1998. Research Methods and Procedures: A sample of 13,089 men and women (age, ≥15 years) were randomly selected and then stratified by province and urban and nonurban areas. Height, weight, mid‐upper arm circumference, and waist and hip circumference were measured. Body mass index (BMI) was use…
Dietary intake and barriers to dietary compliance in black type 2 diabetic patients attending primary health-care services
Objective: To determine the dietary intake, practices, knowledge and barriers to dietary compliance of black South African type 2 diabetic patients attending primary health-care services in urban and rural areas. Design: A cross-sectional survey. Dietary intake was assessed by three 24-hour recalls, and knowledge and practices by means of a structured questionnaire ( n =133 men, 155 women). In-depth interviews were then conducted with 25 of the p…
Where does the black population of South Africa stand on the nutrition transition
Objective: To review data on selected risk factors related to the emergence of non communicable diseases (NCDs) in the black population of South Africa. Methods: Data from existing literature on South African blacks were reviewed with an emphasis placed on changes in diet and the emergence of obesity and related NCDs. Design: Review and analysis of secondary data over time relating to diet, physical activity and obesity and relevant to nutrition-…
Lung cancer, tobacco, and environmental factors in the African population of the Northern Province, South Africa
The data suggest that tobacco smoking is the most important risk factor for the development of lung cancer in this setting. Risks for lung cancer are reminiscent of those observed in Western countries in the 1960s and 1970s. However, environmental exposure to asbestos, a dusty occupation (in men), and perhaps indoor air pollution may also contribute to the development of lung cancer in this province
An inverse relation between blood pressure and birth weight among 5 year old children from Soweto, South Africa
STUDY OBJECTIVE: To examine the relation between birth weight and blood pressure at 5 years in a cohort of South African children. DESIGN: Prospective cohort study. PARTICIPANTS: 849 five year old children. SETTING: Soweto, a sprawling urban area close to Johannesburg, South Africa, which was a designated residential area for people classified as "black" under apartheid legislation. Plasma fibrinogen of black South Africans
Objective: To describe the distribution of plasma fibrinogen and relationships with other risk factors for coronary heart disease (CHD) and stroke in the black population of the Cape Peninsula. Design: A cross-sectional survey of a stratified proportional sample of randomly selected black men and women. Setting: Households in Gugulethu, Langa, Nyanga, New Crossroads, KTC, Old Crossroads and Khayelitsha in the Cape Peninsula, South Africa. Subject…
Nutrient intake in the coloured population of the Cape Peninsula
A diet survey, using the 24‐hour recall method, was conducted in 1982 as part of a cross‐sectional study of the prevalence of coronary heart disease risk factors among the so‐called coloured population of the Cape Peninsula. Randomly selected male and female respondents between 15 and 64 years of age (n = 976) were interviewed by trained nursing sisters. A high animal protein and fat (37% of total energy) intake reflected a typical Western type d…
