Effect of the Rural Interprofessional Faculty Development Initiative on VHA Clinician Turnover by Level of Program Engagement
The VA Rural Interprofessional Faculty Development Initiative
Cannabis Legalization and Cannabis Use Disorder by Sex in Veterans Health Administration Patients, 2005–2019
BACKGROUND: Understanding sex differences in the effects of cannabis legalization and increasing risk for cannabis use disorder (CUD) is important. We hypothesized that from 2005 to 2019, increases in CUD prevalence due to state medical or recreational cannabis laws (MCL; RCL) would differ among male and female veterans treated at the U.S. Veterans Health Administration (VHA), with greater increases among females. METHODS: Data obtained through t…
Enhancing Rural Health Professions Education
PURPOSE: Rural health professions education (HPE) is essential to mitigating workforce shortages in rural communities. The Department of Veterans Affairs (VA) Rural Interprofessional Faculty Development Initiative (RIFDI) is a multimodal, longitudinal faculty development program designed to enhance teaching and leadership skills of clinician-educators from multiple professions practicing in primarily rural settings. This qualitative evaluation as…
Cannabis Legalization and Opioid Use Disorder in Veterans Health Administration Patients
The results of this cohort study suggest that MCL and RCL enactment was associated with greater OUD prevalence in VHA patients over time, with the greatest increases among middle-aged and older patients and those with chronic pain. The findings did not support state cannabis legalization as a means of reducing the burden of OUD during the ongoing opioid epidemic
Compensation for Veterans With Service Connected Disabilities
Compensation is an essential benefit provided by the Department of Veterans Affairs (VA) for veterans with service connected conditions, which are illnesses or injuries incurred or aggravated during military service. The purpose of this paper is to inform beneficiaries, program administrators, policy makers, and the general public about key developments in the VA compensation program. Based on findings from publicly available data as well as rece…

The Burden of Mental Illness Among Veterans
BACKGROUND: Little is known about how Veterans with service-connected conditions use health care provided by the Veterans Health Administration (VHA). OBJECTIVES: To ascertain what proportion of Veterans with service-connected conditions used VHA health care and whether it varied according to type of condition, combined disability rating, age, sex, military rank, or other characteristics and whether there were differences in receipt of inpatient …
Drug Use Severity, Mortality, and Cause of Death in Primary Care Patients With Substance Use Disorders
This article examines the association between drug use severity and mortality in primary care patients with substance use disorders. In 848 individuals with known vital status, severity was low in 32%, intermediate in 38%, or substantial/severe in 30%. Two-year survival was 93% in the low group, 96% in the medium group, and 92% in the high group ( p = .19). The age-adjusted risk of death for the high-severity group was twice that of the medium-ri…
Prevalence, Comorbidity, and Prognosis of Mental Health Among US Veterans
Objectives. We evaluated the association of mental illnesses with clinical outcomes among US veterans and evaluated the effects of Primary Care–Mental Health Integration (PCMHI). Methods. A total of 4 461 208 veterans were seen in the Veterans Health Administration’s patient-centered medical homes called Patient Aligned Care Teams (PACT) in 2010 and 2011, of whom 1 147 022 had at least 1 diagnosis of depression, posttraumatic stress disorder (PTS…
Longitudinal Investigation of Smoking Initiation and Relapse Among Younger and Older US Military Personnel
Objectives. We examined whether military service, including deployment and combat experience, were related to smoking initiation and relapse. Methods. We included older (panel 1) and younger (panel 2) participants in the Millennium Cohort Study. Never smokers were followed for 3 to 6 years for smoking initiation, and former smokers were followed for relapse. Complementary log-log regression models estimated the relative risk (RR) of initiation an…
Predicting Risk of Hospitalization or Death Among Patients Receiving Primary Care in the Veterans Health Administration
BACKGROUND: Statistical models that identify patients at elevated risk of death or hospitalization have focused on population subsets, such as those with a specific clinical condition or hospitalized patients. Most models have limitations for clinical use. Our objective was to develop models that identified high-risk primary care patients. METHODS: Using the Primary Care Management Module in the Veterans Health Administration (VHA)'s Corporate Da…
Annual Rescreening for Alcohol Misuse
BACKGROUND: Routine alcohol screening is widely recommended, and Medicare now reimburses for annual alcohol screening. Although up to 18% of patients will screen positive for alcohol misuse, the value of annual rescreening for patients who repeatedly screen negative is unknown. OBJECTIVE: To evaluate the probability of converting to a positive alcohol screen at annual rescreening among VA outpatients who previously screened negative 2-4 times. RE…

The Reliability of Medical Group Performance Measurement in a Single Insurer’s Pay for Performance Program
BACKGROUND: Most public reporting and pay for performance (P4P) programs in the United States continue to be organized and implemented by single insurers. Adequate medical group-level reliability on clinical care process measures is possible in multistakeholder initiatives because patient samples can be pooled across payers. However, the extent to which reliable measurement is achievable in single insurer P4P initiatives remains unclear. METHODS:…
Risk Factors for Nonfatal Overdose at Seattle-Area Syringe Exchanges
Use of Administrative Claims Models to Assess 30-Day Mortality Among Veterans Health Administration Hospitals
BACKGROUND: The Centers for Medicare and Medicaid Services (CMS) publicly reports hospital-specific risk-standardized, 30-day, all-cause, mortality rates (RSMRs) for all hospitalizations among fee-for-service Medicare beneficiaries for acute myocardial infarction (AMI), heart failure (HF), and pneumonia at non-Federal hospitals. OBJECTIVE: To examine the performance of the statistical models used by CMS among veterans at least 65 years of age hos…
Two Brief Alcohol-Screening Tests From the Alcohol Use Disorders Identification Test (Audit)
BACKGROUND: Primary care physicians need a brief alcohol questionnaire that identifies hazardous drinking and alcohol use disorders. The Alcohol Use Disorders Identification Test (AUDIT) questions 1 through 3 (AUDIT-C), and AUDIT question 3 alone are effective alcohol-screening tests in male Veterans Affairs (VA) patients, but have not been validated in women. METHODS: Female VA patients (n = 393) completed self-administered questionnaires, inclu…
Race, Treatment, and Survival of Veterans With Cancer of the Distal Esophagus and Gastric Cardia
CONTEXT: Prior studies have found racial differences in the use of invasive procedures and in cancer survival. OBJECTIVE: To assess the influence of race on the treatment and survival of patients with distal esophageal cancer. DESIGN: Retrospective cohort study. SETTING: All Veterans Affairs Medical Centers. PATIENTS: One thousand two hundred ninety white and 231 black male veterans with a new diagnosis of distal esophageal cancer during 1993 to …

The Welfare State within the Military
Much has been written concerning the extent to which contemporary industrialized societies meet the accepted criteria of a "welfare state". By contrast, the literature on the welfare aspects of the military as an institution within societies is comparatively sparse. Yet internally, military establishments often exhibit many welfare state attributes. Within the military's organizational territory and authority, members are provided with a wide spe…
