Does It Matter Where You Go for Breast Surgery
BACKGROUND: Concerns about the use of mastectomy and breast reconstruction for breast cancer have motivated interest in surgeon's influence on the variation in receipt of these procedures. OBJECTIVES: To evaluate the influence of surgeons on variations in the receipt of mastectomy and breast reconstruction for patients recently diagnosed with breast cancer. METHODS: Attending surgeons (n = 419) of a population-based sample of breast cancer patien…
Coordinating Cancer Care
OBJECTIVES: The Institute of Medicine has called for more coordinated cancer care models that correspond to initiatives led by cancer providers and professional organizations. These initiatives parallel those underway to integrate the management of patients with chronic conditions. METHODS: We developed 5 breast cancer patient and practice management process measures based on the Chronic Care Model. We then performed a survey to evaluate patterns…
The Influence of Race, Ethnicity, and Individual Socioeconomic Factors on Breast Cancer Stage at Diagnosis
Objectives. Previous research has generally found that racial/ethnic differences in breast cancer stage at diagnosis attenuate when measures of socioeconomic status are included in the analysis, although most previous research measured socioeconomic status at the contextual level. This study investigated the relation between race/ethnicity, individual socioeconomic status, and breast cancer stage at diagnosis. Methods. Women with stage 0 to III b…
Correlates of Between-Surgeon Variation in Breast Cancer Treatments
BACKGROUND: Determinants of between-surgeon variation in breast cancer treatment utilization are not well understood. OBJECTIVES: The objectives of this study were to evaluate variation in receipt of surgical treatment (ie, mastectomy or breast-conserving surgery with or without radiation) for women with stage I, II, or III breast cancer and receipt of breast reconstruction attributable to surgeons, and to assess factors associated with this betw…
Population-based study of the relationship of treatment and sociodemographics on quality of life for early stage breast cancer
Breast Cancer Treatment Experiences of Latinas in Los Angeles County
Objective. We examined breast cancer treatment experiences of and outcomes for Latinas in Los Angeles County. Methods. Informal Caregiving for Diabetes and Diabetic Complications Among Elderly Americans
Diabetes imposes a substantial burden on elderly individuals, their families, and society, both through increased rates of disability and the significant time that informal caregivers must spend helping address the associated functional limitations. Future evaluations of the costs of diabetes, and the cost-effectiveness of diabetes interventions, should consider the significant informal caregiving costs associated with the disease
Mammography messages in popular media
Objective To examine the relationship between the quantity and content of information about mammography in popular magazines and the educational level of their target audience. Design Articles published in popular magazines from January 1988 through April 1994 in which ≥ 25% of all readers were females ≥ 35 years of age were identified ( n =65). We used the proportion of readers who were college graduates to stratify the magazines into three educ…
The Explosion in Paid Home Health Care in the 1990s
OBJECTIVE: Public expenditures for home health care grew rapidly in the 1990s, but it remains unclear to whom the additional services were targeted. This study tests whether the rapidly increasing expenditures were targeted to the elderly with high levels of disability and low levels of social support, 2 groups that have historically been higher users of paid home health and nursing home services. METHODS: The Asset and Health Dynamics Study, a n…
Physician use in Ontario and the United States
OBJECTIVES: This study compared physician use in Ontario and the midwestern and northeastern United States for persons of different socioeconomic status and health status. The distribution of health problems associated with the most recent physician visit also was compared. METHODS: The design of the study was cross sectional; data derived from the 1990 Ontario Health Survey and the 1990 US National Health Interview Survey were used in analyses. …
Healthy behaviors among women in the United States and Ontario
OBJECTIVES: This study examined how several healthy behaviors among women in Ontario and the United States explained (1) the use of preventive health services, (2) differences in use between socioeconomic groups, and (3) differences in use between the two health systems. METHODS: 1990 data on women from the Ontario Health Survey (n = 22,985) and the US National Health Interview Survey (n = 19,092) were analyzed. A woman who avoided smoking and ob…
Comparing the Use of Diagnostic Tests in Canadian and US Hospitals
Although Americans pay much more for a day in the hospital than Canadians, we know little about whether inpatient physician practice patterns might explain some of this difference. The authors compared the utilization of all diagnostic imaging (plain radiographs, computed tomography (CT) and magnetic resonance imaging (MRI) scanning, ultrasound, nuclear medicine and vascular studies) and selected laboratory tests (hematology, basic biochemistry, …
The adequacy of prenatal care and incidence of low birthweight among the poor in Washington State and British Columbia
OBJECTIVES. The purpose of this study was to examine differences in adequacy of prenatal care and incidence of low birthweight between low-income women with Medicaid in Washington State and low-income women with Canadian provincial health insurance in British Columbia. METHODS. A population-based cross-sectional study was done by using linked birth certificates and claims data. RESULTS. Overall, the adjusted odds ratio for inadequate prenatal car…

Trends in the Utilization of Mammography in Washington State and British Columbia
To compare the utilization of an imaging technology in the United States and Canada and its effect on clinical outcomes, trends in the utilization of mammography among women aged 50 to 75 years from 1984 through 1988 in Washington State and British Columbia were examined. Also compared were trends in the stage at time of diagnosis of invasive breast cancer and mortality due to breast cancer in the two regions. Annual mammography use increased in …
