Open Access•Woan Shin Tan, Jonathan Gao et al.•ARTICLE•Frontiers in Public Health•2025

Early detection and assessment of geriatric syndromes and social issues is important to help older adults maintain health and functional ability. While comprehensive geriatric assessment (CGA) is considered the gold standard, it is resource intensive to implement. Shorter forms such as a brief geriatric assessment (BGA) could be an alternative. We adopted a multi-method, three-phase study to understand how BGAs could be implemented in the communi…

  • Development and validation of the multi-dimensional health resilience scale for community-dwelling adults

Open Access•Lixia Ge, Wan Fen Yip et al.•ARTICLE•Frontiers in Public Health•2025

Introduction: Resilience measures generally are not health specific, nor do they account for the multiple dimensions required for individuals to overcome health challenges. To bridge this gap, we developed and validated a multi-dimensional Health Resilience Scale (HRS) for community-dwelling adults in Singapore. Methods: We followed standard procedures to develop health resilience construct, identify dimensions, and generate potential items. Expe…

  • Lived Experiences and Coping Strategies of Unpaid Informal Caregivers of Community-Dwelling Older Adults in Singapore

Open Access•Jonathan Gao, Ze Ling Nai et al.•ARTICLE•International Journal of…•2025

Singapore’s population is rapidly ageing, and the city-state is expected to be a super-aged society by 2030. The difficulties and experiences of caregivers of older adults are gaining attention, and various measures to support them have been implemented. Yet, caregivers continue to face challenges ranging from financial difficulties to mental and emotional issues. Qualitative research can provide in-depth understanding on challenges faced by care…

  • Communities of Care Approach

Open Access•Wei Ting Chen, Sing YONG LIM et al.•ARTICLE•International Journal of…•2024•Referências: 2

The population in Singapore is ageing, adding pressure to community care as the health and social needs of its residents increase. This has accelerated the pace at which Regional Health Systems adopt and deliver its population health strategies from early prevention, chronic disease management, crisis care to end-of-life care. To this end, the Central Health Integrated Care Network (ICN) began its journey to develop Communities of Care (CoCs) wit…

  • Perceived Roles and Barriers in Delivering Community-Based Care

Open Access•Lixia Ge, Wan Fen Yip et al.•ARTICLE•International Journal of…•2023•Referências: 2

Our results highlight that the roles of HCPs and SCPs go beyond the provision of direct medical and social care. They were involved in activating and empowering clients to take care of their health, and importantly, fostering community-based sustainable support networks to better empower individuals in coping with health challenges. The identified barriers shed light on areas for potential improvements for integrated community care

  • Global Trends Towards Population Health Management and Key Lessons and Initiatives in the Singapore Context

    Open Access•Maria Abraham, H G Schotman et al.•ARTICLE•International Journal of…•2022

  • Competing expectations

Priya Lall, Natalie Kang et al.•ARTICLE•Death Studies•2020•Referências: 2

This study qualitatively examines the perspectives of doctors and nurses on the implementation of the Advance Care Planning program in Singapore. Findings suggest that a combination of structural and conceptual factors hindered the performance of ACP. Themes on structural factors indicated that low awareness of ACP among senior staff resulted overall lack of buy-in and incorrect implementation of the program due to misconceptions. Conceptual fact…
