Leveraging Community Support Services to Support an Integrated Health and Social System Response to Covid-19
The COVID-19 pandemic restricted access to health and social care for older adults. In response, a standardized, self-report instrument, the interRAI COVID-19 Vulnerability Screener (CVS), was developed. Introduction: In Canada, long-stay home care patients are a prevalent group of frail older adults living in the community. Home care patients have a high emergency department visit rate compared to those living independently without formal home care services. Implementing the interRAI Check-Up Comprehensive Assessment
The Check-Up was advantageous in providing sufficient information to create a care plan when administered over the phone and by video. Implementation of the Check-Up assessment was facilitated by familiarity and alignment with other interRAI assessments. Our results indicate that population characteristics need to be taken into consideration for administration of self-report style of assessments
