Development of a Layperson Injector Training for the Home-Based Administration of Long-Acting Injectable Antiretroviral Therapies for Persons With HIV
Long-acting injectable antiretroviral therapy (LAI-ART) has the potential to transform HIV treatment by improving adherence, reducing stigma, and expanding access through decentralized care. The INVITE-Home study evaluates home-based LAI-ART administration by trained nonmedical treatment buddies (TBYs) selected by people with HIV (PWH). This article describes the development of a TBY training curriculum guided by Adult Learning Theory and refined…
Influence of alcohol-serving venue characteristics on yield of HIV status-neutral screening in rural East Africa
Alcohol-serving venues are high-risk sites for HIV transmission in East Africa. Understanding how venue characteristics influence HIV screening outcomes may help to target venue-based outreach. In eight rural communities in Kenya and Uganda, we mapped all alcohol-serving venues (N = 530) and invited owners to participate in a cluster-randomized trial to promote biomedical HIV prevention uptake: 527 (99%) owners agreed to participate. We distribut…
Cost Considerations for Rapid Start Implementation
Antiretroviral treatment (ART) initiation within days of HIV diagnosis, also known as “Rapid Start”, improves linkage to care, virologic suppression and retention in care. However, data on the costs of Rapid Start implementation in the United States are limited. We aimed to estimate the costs and cost-effectiveness of Rapid Start among seven geographically diverse Ryan White-funded programs with broad variability in populations served and service…
Association between Levels of Alcohol Use, Perceived HIV Risk, and Untreated HIV Among Persons at Alcohol-Serving Venues in Kenya and Uganda
Alcohol-serving venues in East Africa are high-risk environments for HIV transmission but few studies in sub-Saharan Africa (SSA) have examined perceived HIV risk and sexual risk behaviors by alcohol use levels on a broader population of persons at alcohol-serving venues in the era of biomedical HIV prevention. We conducted a cross-sectional analysis of adults (≥ 18 years) at alcohol-serving venues in eight communities (~ 10,000 residents/communi…
Facilitators and Barriers to Home-Based Long-Acting Injectable Antiretroviral Therapy
Measuring Implementation Outcomes Change Over Time Using an Adapted Checklist for Assessing Readiness to Implement (Cari)
In 2017, the Health Resources and Services Administration's HIV/AIDS Bureau funded an Evaluation Center (EC) to assess the rapid implementation of 11 evidence-informed interventions at 25 HIV care and treatment providers across the U.S. The EC conducted an implementation science-based evaluation, including longitudinal assessment of implementation outcomes as defined by Implementation Outcome Framework (IOF) of the Conceptual Model for Implementa…
User-informed tailoring of electronic and in-person peer navigation strategies to improve retention in HIV care for adolescents and young adults
Poor adherence and retention in HIV care remain a major challenge among adolescents and young adults (AYA) living with HIV in sub-Saharan Africa (SSA). Strategies are urgently required to support AYA to remain in care for better health outcomes. We explored AYA preferences regarding the format and delivery of electronic and in-person peer navigation to improve HIV care outcomes. This formative qualitative study was conducted among AYA enrolled in…
A guaranteed income intervention to improve the health and financial well-being of low-income black emerging adults
Background: Economic inequity systematically affects Black emerging adults (BEA), aged 18-24, and their healthy trajectory into adulthood. Guaranteed income (GI)-temporary, unconditional cash payments-is gaining traction as a policy solution to address the inequitable distribution of resources sewn by decades of structural racism and disinvestment. GI provides recipients with security, time, and support to enable their transition into adulthood a…
Cost comparison of a rapid results initiative against standard clinic-based model to scale-up voluntary medical male circumcision in Kenya
Voluntary male medical circumcision (VMMC) reduces HIV acquisition by up to 60%. Kenya has successfully scaled up VMMC to an estimated 91% of eligible men and boys in certain regions in combination due to VMMC and cultural circumcisions. VMMC as a program is implemented regionally in traditionally non-circumcising counties where the prevalence is still below 91%, ranging from 56.4% to 66.7%. Given that funding toward VMMC is expected to decline i…
Costs of a Brief Alcohol Consumption Reduction Intervention for Persons Living with HIV in Southwestern Uganda
Low-cost interventions are needed to reduce alcohol use among persons with HIV (PWH) in low-income settings. Brief alcohol interventions hold promise, and technology may efficiently deliver brief intervention components with high frequency. We conducted a costing study of the components of a randomized trial that compared a counselling-based intervention with two in-person one-on-one sessions supplemented by booster sessions to reinforce the inte…
Improvements in pediatric and adolescent HIV testing and identification in western Kenya under the Accelerating Children’s HIV/Aids Treatment initiative
Pediatric HIV remains a significant global concern, with 160,000 new infections annually. Accelerating Children’s HIV/AIDS Treatment (ACT) provided a strategic response to the “treatment gap” for children. We examined whether activities under ACT increased testing and identification of youth living with HIV (YLWH). Family AIDS Care & Education Services implemented ACT across 130 health facilities in western Kenya between October 2015 and Septembe…
Implementation Science Protocol
In 2017, the Health Resources and Services Administration's HIV/AIDS Bureau funded an Evaluation Center (EC) and a Coordinating Center for Technical Assistance (CCTA) to oversee the rapid implementation of 11 evidence-informed interventions at 26 HIV care and treatment providers across the U.S. This initiative aims to address persistent gaps in HIV-related health outcomes emerging from social determinants of health that negatively impact access t…
Relationship, partner factors and stigma are associated with safer conception information, motivation, and behavioral skills among women living with HIV in Botswana
Knowledge of SC methods varied widely. While some SC methods were well known (medical male circumcision by 83%, antiretroviral therapy for viral suppression by 64%), most other methods were known by less than 40% of participants. Our final models reveal that stigma as well as relationship and partner factors affect SC information, motivation, and behavioral skills. Both internalized childbearing stigma (ß=-0.50, 95%CI:-0.17, -0.02) and perceived …

Integrating family planning and HIV services in western Kenya
Little information exists on the impact of integrating family planning (FP) services into HIV care and treatment on patients' familiarity with and attitudes toward FP. We conducted a cluster-randomized trial in 18 public HIV clinics with 12 randomized to integrated FP and HIV services and 6 to the standard referral-based system where patients are referred to an FP clinic. Serial cross-sectional surveys were done before (n = 488 women, 486 men) an…
Helping clinicians deliver consistent HIV prevention counseling to their HIV-infected patients
The delivery of HIV risk assessment and behavioral counseling by clinicians in HIV clinical settings is one component in a comprehensive "positive prevention" strategy to help patients reduce their transmission risk behavior. Clinicians engage in behavioral prevention inconsistently, however, depending on whether patients are new to a practice or are established in regular care and on their attitudes and characteristics of their practices. We ana…
Messages HIV clinicians use in prevention with positives interventions
Prevention with Positives (PwP) is a component of the US HIV prevention strategy that targets HIV-infected persons who are aware of their seropositive status. This paper examines the use of prevention messages by clinical providers during the PwP intervention period of the US Health Resources and Services Administration's Special Projects of National Significance program. Quantitative approaches were used to learn which prevention topics were mos…
Predictors of Linkage to Care Following Community-Based HIV Counseling and Testing in Rural Kenya
Interventions Delivered in Clinical Settings are Effective in Reducing Risk of HIV Transmission Among People Living with HIV
To support expanded prevention services for people living with HIV, the US Health Resources and Services Administration (HRSA) sponsored a 5-year initiative to test whether interventions delivered in clinical settings were effective in reducing HIV transmission risk among HIV-infected patients. Across 13 demonstration sites, patients were randomized to one of four conditions. All interventions were associated with reduced unprotected vaginal and/…
