Home-based transcranial direct current stimulation (tDCS) for bipolar depression
A 6-week course of supervised home-based tDCS was associated with significant QoL improvements in bipolar depression, which appeared to be closely linked to reduction in depressive symptoms. Randomized, sham-controlled trials are warranted to clarify the specific contribution of tDCS to improve QoL in bipolar depression
Nomogram for estimating the risk of suicide attempts in major depressive disorder
Several routinely available variables, including tobacco use, alcohol consumption, education level, life events, and folate level, were associated with suicide attempts in MDD inpatients. Our findings highlight these important correlates, which may help clinicians in recognizing patients at elevated risk. The nomogram provided in this study should be regarded only as a visualization to better illustrate the contribution of these factors, rather t…
The lived experience of major and treatment-resistant depression in England
INTRODUCTION: Major depressive disorder (MDD) is a common, frequently recurrent condition associated with decreased well-being and increased healthcare-related costs. Mixed-methods research provides multiple ways of illustrating the phenomenon to better understand patient experience, including where treatment is not working, referred to here as treatment-resistant depression (TRD). METHODS: A mixed-methods study investigated the experiences of pe…
Single-Dose Psilocybin for a Treatment-Resistant Episode of Major Depression
BACKGROUND: Psilocybin is being studied for use in treatment-resistant depression. METHODS: In this phase 2 double-blind trial, we randomly assigned adults with treatment-resistant depression to receive a single dose of a proprietary, synthetic formulation of psilocybin at a dose of 25 mg, 10 mg, or 1 mg (control), along with psychological support. The primary end point was the change from baseline to week 3 in the total score on the Montgomery-Å…
Variants in the zinc transporter-3 encoding gene (SLC30A3) in schizophrenia and bipolar disorder
Zinc transporter 3 (ZnT3) has been implicated in the aetiopathology of schizophrenia. In this pilot study, we tested the hypothesis that the presence of a minor allele of two variants in the gene encoding ZnT3 (SLC30A3) affects brain glutamate and cognitive activity in patients with schizophrenia and bipolar affective disorder. Fifteen patients with schizophrenia (SCZ), 15 with bipolar affective disorder type 2 (BD), and 14 healthy volunteers (HV…
Sitagliptin for Depressive Symptoms in Type 2 Diabetes
OBJECTIVE: We tested the feasibility of using sitagliptin-a dipeptidyl peptidase-IV inhibitor-for depressive symptoms in type 2 diabetes (T2D). METHODS: In a feasibility, double-blind, randomized controlled trial, we recruited people aged 18 to 75 years with T2D (glycated hemoglobin A1c levels ≥53 and ≤86 mmol/mol prescribed oral hypoglycemic therapy) and comorbid depressive symptoms (Patient Health Questionnaire-9 score ≥10) from family practice…
A Novel Group Cognitive Behavioral Therapy Approach to Adult Non-rapid Eye Movement Parasomnias
Background: Following the success of Cognitive Behavioral Therapy (CBT) for insomnia, there has been a growing recognition that similar treatment approaches might be equally beneficial for other major sleep disorders, including non-rapid eye movement (NREM) parasomnias. We have developed a novel, group-based, CBT-program for NREM parasomnias (CBT-NREMP), with the primary aim of reducing NREM parasomnia severity with relatively few treatment sessi…
Ceremonial Ayahuasca in Amazonian Retreats—Mental Health and Epigenetic Outcomes From a Six-Month Naturalistic Study
Ayahuasca is a natural psychoactive brew, used in traditional ceremonies in the Amazon basin. Recent research has indicated that ayahuasca is pharmacologically safe and its use may be positively associated with improvements in psychiatric symptoms. The mechanistic effects of ayahuasca are yet to be fully established. In this prospective naturalistic study, 63 self-selected participants took part in ayahuasca ceremonies at a retreat centre in the …
Psilocybin
Psilocybin has a long history of non-medical use and some seem to infer from this that it has therapeutic utility. Early phase clinical trials with psilocybin are encouraging, but suggest only that larger, multicentre trials are required. These are ongoing but will take many years to complete. Meanwhile, retreat centers offering paid experiences with psilocybin truffles have opened in some countries, often using early phase clinical trial data as…
Promotion of Wellbeing for Children of Parents With Mental Illness
Background: The main objective of this project is to create a research and intervention model to promote large-scale implementation and evaluations of generic very brief interventions for children of parents with mental disorders (COPMI). Feasible interventions for COPMI aged 0-18 years are highly needed, as this is a large high-risk group in society. Reducing behavioral problems and enhancing wellbeing for families with parents affected by any m…
Inflammation, Glutamate, and Cognition in Bipolar Disorder Type II
Background: Two current theories regarding the neuroscientific bases of mood disorders involve alterations in glutamatergic neurotransmission and excessive activation of inflammatory pathways. We hypothesized that glutamate (Glu) levels and peripheral inflammatory markers would be associated with cognitive function, in patients with Bipolar Disorder Type II (BP-II), and that such factors would be associated with psychological treatment outcomes. …
Allan V. Horwitz, PTSD, A Short History
In this book, Allan Horwitz provides a superb compact history of post-traumatic stress disorder (PTSD). Throughout, his writing is lucid and coverage is comprehensive: nearly all significant contributors to the history of PTSD research are mentioned, often together with abbreviated accounts of significant or emblematic research. His book effectively challenges interpretations of PTSD that reduce its history to a discourse on victimhood and suffer…
Growth Factor Proteins and Treatment-Resistant Depression
Background: Since the neurotrophic hypothesis of depression was formulated, conflicting results have been reported regarding the role of growth factor proteins in depressed patients, including whether there are state or trait alterations found in patients compared to controls and whether they represent predictors of treatment response. Recently it has been hypothesized that heterogeneity of findings within this literature might be partly explaine…

The Harmony of Illusions
As far back as we know, there have been individuals incapacitated by memories that have filled them with sadness and remorse, fright and horror, or a sense of irreparable loss. Only recently, however, have people tormented with such recollections been diagnosed as suffering from "post-traumatic stress disorder." Here Allan Young traces this malady, particularly as it is suffered by Vietnam veterans, to its beginnings in the emergence of ideas abo…

Some Implications of Medical Beliefs and Practices for Social Anthropology
A people's medical beliefs and practices persist because they answer instrumental and moral imperatives, and they are empirically effective. This is not the same thing as saying that they are effective from the standpoint of Western medical notions or that they always bring the results for which people hope. The empirical effectiveness of these practices has important ontological consequences, since it enables sickness episodes to communicate and…
The McGill Illness Narrative Interview (Mini)
This article summarizes the rationale, development and application of the McGill Illness Narrative Interview (MINI), a theoretically driven, semistructured, qualitative interview protocol designed to elicit illness narratives in health research. The MINI is sequentially structured with three main sections that obtain: (1) A basic temporal narrative of symptom and illness experience, organized in terms of the contiguity of events; (2) salient prot…

Why Amhara get kureynya
Most accounts of spirit possession cults treat illnesses caused by spirits as an ancillary concern, interesting mainly as events through which people join such cults. However, in the Amhara zar cult, sicknesses caused by spirits remain an important focus for relations between cult members and cult spirits even after initiation. Further, it is by participating in recurrent sickness episodes that members gain access to the adaptive strategies offer…
Urban Regimes and Racial Politics in New Orleans
This paper describes and analyzes the nature of the New Orleans urban regime focusing on the first term of Sidney Barthelemy, the second black mayor of New Orleans (1986–1990). First, because the authors believe that regime theory needs to be more carefully theorized in relation to racial politics, they begin by discussing the changing relationship of blacks to the prevailing New Orleans regime. Second, in analyzing this racial politics, they foc…
"Magic as a "Quasi-Profession
In this paper, I describe how certain Ethiopian quasi-professionals, debtera magician-healers, monopolize a highly volatile resource-knowledge, while lacking either a ritual articulation or corporate infrastructure (as do castes, bureaucracies, and true professions), and how they control the destribution of this resource among themselves and prevent it from evaporating beyond their ranks. I make two general points. Recruitment to this quasi-profe…
Reasons and Causes for Post-Traumatic Stress Disorder
PTSD's most distinctive feature is its etiological event. With out this event, PTSD symptoms are indistinguishable from symptoms associated with combinations of other psychiatric diagnoses. The DSM definition of PTSD assumes that the relation between the etiological event and its symptoms is equivalent to a cause and effect. In reality, these events are often reasons rather than causes of the syndrome. Where this is true, the syndrome loses its s…
Unpacking the Demoralization Thesis
American psychotherapies use a variety of explanations for their practices. From an anthropologist's point of view, one of the most sophisticated of these explanations is the demoralization thesis proposed by the psychiatrist Jerome Frank. Frank's thesis is that (1) no psychotherapy has been shown to be more efficacious than any other psychotherapy, and (2) claims about the special efficacy and validity of particular traditions and techniques are…
The Social Brain and the Myth of Empathy
ArgumentNeuroscience research has created multiple versions of the human brain. The "social brain" is one version and it is the subject of this paper. Most image-based research in the field of social neuroscience is task-driven: the brain is asked to respond to a cognitive (perceptual) stimulus. The tasks are derived from theories, operational models, and back-stories now circulating in social neuroscience. The social brain comes with a distincti…

Remembering the Evolutionary Freud
ArgumentThroughout his career as a writer, Sigmund Freud maintained an interest in the evolutionary origins of the human mind and its neurotic and psychotic disorders. In common with many writers then and now, he believed that the evolutionary past is conserved in the mind and the brain. Today the "evolutionary Freud" is nearly forgotten. Even among Freudians, he is regarded to be a red herring, relevant only to the extent that he diverts attenti…
Our Traumatic Neurosis and Its Brain
Argument During the nineteenth century, physicians either discovered or invented a variety of clinical autobiography called "traumatic memory." Freud produced two versions of this memory, the final version in the 1920s. A revolutionary nosology (DSM-III), adopted in 1980, promised to extirpate Freud and the concept of neurosis from American psychiatry. However, it made a tacit exception for Freud's concept of traumatic neurosis, renaming it "post…

Evolutionary narratives about mental disorders
There is a growing interest, among psychiatrists and psychologists, in the evolutionary origins of clinical syndromes. Evolutionary explanations are organized around stories that connect syndromal features to environmental adaptations. The stories are based on a 'modular' vision of mind and human nature. This paper considers the credibility of evolutionary narratives intended to explain depression and homosexuality.|spagf|ro|epagf
