Vaccine safety
Vaccines are among the most cost-effective health interventions. Scientifically rigorous monitoring and evaluation of vaccine safety is critical in order to develop and modify recommendations for vaccine use and in maintaining public trust necessary for the use of vaccines. Even when governments and public health advocates are appropriate in their enthusiasm, vaccines sometimes lack advocates among caregivers or citizens. Outside of pandemics, va…
Active Influenza Vaccine Safety Surveillance
BACKGROUND: Rapid safety assessment of novel vaccines, especially those targeted against pandemic influenza, is a public health priority. OBJECTIVES: Assess the feasibility of using healthcare claims data to rapidly detect influenza vaccine adverse events using sequential analytic methods. RESEARCH DESIGN: Retrospective pilot study simulating prospective surveillance using 6 cumulative monthly administrative claims data extracts. The first includ…
Use of Propensity Score Technique to Account for Exposure-Related Covariates
BACKGROUND: In observational research, propensity score techniques can be used to account for baseline differences between compared therapies. Although propensity scores are used increasingly often, their limitations in settings without complete data may not be recognized. OBJECTIVES: We sought to evaluate the ability of propensity score matching to mitigate confounding by indication in an observational study of the effect of statin therapy on ac…
Multiple sclerosis in the Orkney and Shetland Islands. II
In Orkney and Shetland, a survey of lifetime events was undertaken in multiple sclerosis patients and two control groups to define shared exposure to an exogenous agent or environmental insult. Analyses of demographic factors, diet, social class and occupation, housing and environment, animal exposure, schooling, travel, infectious disease, and medical history disclosed a remarkable similarity in responses between patients and controls for a majo…
The epidemic of endometrial cancer
Vital statistics show that a rise in incidence of endometrial cancer began in the mid-1960s on the West Coast of the United States. This rise was continuous and reached a peak in 1975. Elsewhere, incidence rates for endometrial cancer rose during the 1970s. It now seems evident that much of the rise in all areas of the country was due to replacement estrogen treatment. We estimated from data obtained from the Commission on Professional and Hospit…
