Intervention approaches to address allostatic load-associated health disparities across high income and low/middle- income countries

Objectives: The allostatic load (AL) framework provides a theoretical basis for elucidating how chronic stress contributes to adverse health outcomes and persistent disparities. Despite established theory, the extent to which AL theory has informed formal intervention approaches—and reviewed for representation across high-income countries (HIC) and low/middle-income countries (LMIC)—remains poorly characterized. We undertook a scoping review employing a quaternary methodological approach to broadly map existing evidence and gaps on this topic.Methods: Primary and secondary analyses utilized concept mapping to identify key terminology. Tertiary analysis examined publications by three subject-matter experts. Quaternary analysis synthesized findings to determine allostatic load intervention approaches across published literature.Results: This methodology yielded 18 articles representing diverse intervention approaches across multiple study designs and contextual settings. Intervention themes related to physical and mental health, pharmaceutical treatments, individualized and community-based prevention and management strategies. While health disparities were referenced, attention through defined disparity reduction was not explicit across intervention profiles.Conclusions: Findings of this scoping review highlight gaps in equitable upstream, AL health disparity intervention approaches in literature across HIC and LMIC settings. Future research should incorporate modern methodological approaches using artificial intelligence (AI) with attention to strategies that mitigate global burden of disease and mortality attributable to chronic stress
