OBJECTIVE: To analyze the role of primary care (PC) as the backbone of care coordination, with emphasis on family medicine and nursing. To explore integrated care models as a reference to overcome fragmentation. METHOD: A narrative technical review aimed at describing and analyzing care coordination between PC and hospital care, identifying conceptual frameworks, tools, and relevant experiences. The literature search was conducted in PubMed and Scopus, as well as in technical reports. RESULTS: Three key elements of coordination were identified: continuity of information, communication among professionals, and interpersonal relationships based on trust. The role of family medicine as a coordination hub is highlighted, along with the relevance of nursing, including the case management nurse. Clinical pathways, continuity of care reports, and digital technologies can improve coordination, especially when involving both professionals and patients. CONCLUSIONS: Improving coordination requires complex management models such as integrated care, with PC as a central hub, interoperable information systems, and a person-centred approach. Collaboration among professionals, institutional commitment, and systematic evaluations of these models by regional health services are essential to move toward more efficient, equitable, and needs-based care
