Should management of sexually transmitted infections be integrated into family planning services

An extensive review of what is known about efforts to integrate prevention and treatment services for sexually transmitted infections (STIs) into family planning services found that all too little empirical evidence is available. This paper summarises the key findings of the review, discusses the need for additional information on forms of integration and how they are working, and makes recommendations on gathering the type of information that will allow countries to decide whether and how to integrate these two public health services for women. One area of need is operations research, in which interventions to improve reproductive health through the provision of integrated services can be explored and evaluated. Another important focus is on modelling and costing studies that could help to clarify under what conditions integration of family planning and STI services, and what type of integration, maybe mostbeneficial in specific settings and for specific clinic populations. Further, it highlights the need for descriptive studies of examples of integration that are already in place or developing, several of which are in progress. Finally, the paper argues that even when full programmatic integration is undesirable or not planned, some specific activities that can mutually reinforce the goals of both family planning and STI/HIV programmes can be undertaken. Among these, the authors plead for urgent attention to one specific task of integration in family planning services — the promotion of dual protection among those at risk ofboth STI/HIV and unwanted pregnancy, an important task of any integrated FP/STI/HIV service provision.RésuméUne analyse approfondie de ce que l'on sait des tentatives pour intégrer la prévention des MST et les services de traitement dans les services de planification familiale a révélé que l'on dispose de trop peu de données empiriques. Cet article récapitule les conclusions clefs de l'analyse, discute de la néssitd́'un complént d'information sur les formes d'intégration et sur la maniére dont elles fonctionnent, et formule des recommandations sur la collecte de donnś propres à permettre aux pays de décider s'ils souhaitent intégrer ces deux services de santé publique pour les femmes et comment il convient de le faire. Un domaine de manque est la recherche opérationnelle, qui pent étudier et évaluer les interventions pour améliorer la santé génésique par la prestation de services intégrés. Une autre priorité concerne la définition de modèles et les études de coût qui pourraient aider à clarifier dans quelles conditions l'íntégration des services de planification familiale et de prévention des MST, et quels types d'intégration, seraient les plus bénéfiques dans des environnements précis et pour des populations cliniques concrètes. De plus, l'article met en lumière la nécessité d'études descriptives portant sur des exemples d'intégration qui sont déjà en place ou se développent, dont plusieurs sont en cours. Enfin, l'article avance que, même quand une intégration totale des programmes n'est pas souhaitable ou planifiée, il est possible d'entreprendre certaines activités spécifiques qui peuvent renforcer mutuellement les objectifs des programmes de planification familiale et de prévention des MST/VIH. A cet égard, les auteurs préconisent d'envisager sans délai une tâche précise d'intégration dans les services de planification familiale — la promotion d'une double protection des groupes à risque contre les MST/VIH et les grossesses non désirées, un volet important de tout service intégré de planification familiale, de prévention des MST et du VIH.ResumenUna revisión amplia del conocimiento acerca de los esfuerzos por integrar los servicios de prevención y tratamiento de las ETS en los servicios de planificación familiar reveló que existe poca evidencia empírica al respecto. El presente trabajo resume los hallazgos claves del estudio y plantea la necesidad de más información acerca de las formes de integración y su funcionamiento. También hace recomendaciones en cuanto al recogimiento del tipo de información que permita a los países decidir si deben integrar estos dos servicios de salud pública para la mujer, y cómo hacerlo. Por un lado, se requieren investigaciones que examinan y evalúan las intervenciones destinadas a mejorar la salud reproductiva a través de la provisión de servicios integrados. Por otro, es importante enfocar en los estudios de modelos y costos que ayudarían a identificar los tivos de integración que sean más beneficiosos, y las condiciones bajo las cuáles la integración sea más beneficiosa en lugares específicos y para poblaciones clínicas específicas. Subraya la necesidad de estudios descriptivos de ejemplos de integración que ya están en funcionamiento o en desarrollo. Finalmente, el trabajo plantea que, aun cuando la plena integración programática no es deseada ni planificada, se pueden emprender ciertas actividades específicas que fortalecen mutuamente los objetivos de los programas de planificación familiar y Ids programas de ETSNIH. Los autores ruegan que se preste atención urgente a una tarea de integración específica: la promoción de la protección doble entre las personas que corren el riesgo tanto de ETSNIH como del embarazo no deseado
