first relates to the research reviewed and the second to the philosophical and policy implications.Dr. Westermeyer correctly cautions us about the 15 per cent negative findings, Hawthorne effects, and other unknown factors. There are a number of other important unanswered methodological questions. The second issue is the standard of the art of care to be expected from attending physicians. The literature re- view described unusual interventions which would require a prescription, special staff, and charge to the patient. However, outcome and rehabilitation research repeatedly emphasize the crit- ical role of the physician responsible for care. Are not caring and counseling to be valued, expected, and encour- aged from these physicians?I do not believe we would seriously suggest that ancillary or consultative personnel are needed in routine treatment.Length of stay and other outcomes are clearly sensitive to this aspect of care; that is not the question. The issue is the best, most effective, most likely and cost beneficial means of training and encouraging attending physicians to provide psychosocial support and education
