Hormonal treatment for transgender individuals is associated with certain endocrine tumors. Here we report on a male-to-female transgender patient with a right, 5.5-cm adrenal pheochromocytoma. This is the first report of pheochromocytoma in transgender individuals. A high index of clinical suspicion needs to be maintained in the transgender population for early recognition of this rare tumor
