Introduction: Despite many achievements in the past decade, health care systems in Europe are facing some challenges because of the epidemiological, demographic and economic transition in particular the burden of an aging population, which dictates a greater demand for care due to multiple co-morbidities and long term continuous care. This leads to concerns about the rising healthcare costs and cost-efficiency of the provided health services. On the contrary, the technological advances allow patient self-monitoring and self management and provide better linkages between health care providers (e.g. electronic medical records) that aid against the other challenge faced by healthcare system which is service fragmentation. To address those challenges, the World Health Organization launched a new global strategy on people-centred and integrated health-care services (PCIHCS). In addition, the WHO regional office for Europe advocated for more percent-centeredness in its new European health policy framework Health 2020. The PCIHCS has five strategic directions that aim to reorient the model of care. Peoplecentered approach regards patients as self-determining agents in healthcare rather than just passive recipients of treatment. This article provides evidence on the benefits that people-centred and integrated care can bring through the presentation of a case study on an interdisciplinary Medication Adherence Programme at the Community Pharmacy of the Department of ambulatory care & community medicine in Lausanne, Switzerland (Policlinique Mdicale Universitaire, PMU). It is an interdisciplinary programme that combines the use of Medication Event Monitoring System (MEMS), motivational interviewing and feedback loop towards patient and caregivers
