The electronic health records (EHRs) has been recognized as an effective means to measure, monitor, and evaluate cost and quality of care. Current EHRs system has capacity to identify the effects of each individual intervention or health service. However, evidence gaps have existed in terms of the impacts of integrated care. Prior studies also pointed out the barriers of EHRs adoptions including high initial cost, provider resistance, fear of change, concern of patient privacy, and lack of skills. It is thus necessary to identify barriers, overcome challenges, and expand the capacity of EHRs as a goal of people-centered integrated care
