Introduction:Fragmentation is a major challenge for healthcare. The lines of fragmentation run between primary, secondary and tertiary care, between social and health care, between mental and physical care, between different categories of caregivers, Especially the delivery of chronic care suffers as the broad array of necessary care activities crosses the lines of fragmentation.A key driver of care fragmentation is the way tasks are divided within care organisations and networks.Functionally structured organisations and networks are associated with suboptimal quality of care compared to multidisciplinary ones. This study presents an extension of the Care Delivery Value Chain (CDVC) methodology of Porter & Teisberg (2006) with a task division analysis to provide an in-depth description of care fragmentation. The results of a CDVC task division analysis of the full cycle of care for Multiple Sclerosis (MS) in a region in Flanders (Belgium) are shown
