Fertility awareness is crucial for reproductive-age individuals to prevent infertility and to make informed fertility decisions. This systematic review aimed to synthesise evidence on the effects and efficacy of fertility education on people of reproductive age. Four English language databases and three Chinese language databases were searched for randomised controlled trial studies published before 13 January 2025. The Cochrane Collaboration’s risk of bias assessment tool was used for methodological quality assessment. Quantitative analysis was performed using meta-analysis. The certainty of evidence was evaluated utilising the Grading of Recommendations, Assessment, Development, and Evaluation Approach (GRADE). Meta-analysis including 9 studies showed that fertility education significantly changed fertility awareness but not the desired number of children, the desired age of having a first child, and the desired age for having a last child. The number of high-quality fertility education studies should be increased to more accurately analyse the effects of fertility education and identify potential sources of heterogeneity
