Questions have been raised about the prevailing practice of\ninitiating aggressive therapy for very low birth weight infants. Advocates of\nconservative care are concerned about the financial burden to society, the\nquality of life of surviving infants, and the subsequent economic and\nemotional stress for their families. The author refutes these arguments for a\nconservative approach and maintains, moreover, that considerations of social\njustice require initiating aggressive treatment, though without a binding\ncommitment to its continuation.
