BACKGROUND: Vaccine hesitancy is determined by both cultural and contextual factors. OBJECTIVES: To understand if cultural determinants of vaccine hesitancy may be affected by the context of the country of residence, this study compared vaccine hesitancy between native Danish, Arab minority in Denmark, and the Saudi Arabian populations. METHODS: An international cross-sectional online survey was conducted utilizing validated Danish and Arabic translations of the Vaccination Attitudes Examination (VAX) scale. To compare VAX outcomes across the three groups, linear regression was applied. RESULTS: In Denmark, 44 respondents were of Danish and 63 of Arab heritage; 164 individuals responded in Saudi Arabia. The Arab minority respondents in Denmark were the oldest group (mean age 48 years). The highest proportion of females (77%) and COVID-19 unvaccinated individuals (9%) was in Saudi Arabia. A significant linear increase in both COVID-19 and non-COVID-19 vaccines' hesitancy scores was observed across all VAX dimensions, when comparing Danish heritage, Arab minority in Denmark and Saudi Arabian respondents (p<0.001). CONCLUSION: The increase in vaccine hesitancy observed from Danish heritage to Arab minority in Denmark to Saudi Arabian populations suggests that cultural determinants of vaccine hesitancy may be modified by context of the country of residence
