Natural disasters are not gender-neutral, yet age- and sex-disaggregated data using standardised metrics remain scarce. This ecological study compared the health burden of Türkiye's 1999 and 2023 earthquakes using Global Burden of Disease (GBD) data. We analyzed deaths, disability-adjusted life years (DALYs), years of life lost (YLLs), and years lived with disability (YLDs) across 19 age groups. Results showed 17,981 deaths in 1999 and 53,054 in 2023. Females consistently bore a higher burden; female-to-male (F/M) death rate ratios were 1.63 (1999) and 1.42 (2023). Disparities peaked among women aged 15-34, with death rates 3-4 times higher than men. Notably, the F/M YLD rate ratio surged from 1.69 to 4.50, reflecting a widening disability gap. While mortality disparities remained relatively stable over 24 years, disability inequalities worsened significantly. Gender-responsive disaster policies must prioritize young women and move beyond mortality-centric monitoring to address these expanding long-term health inequities
