In this chapter, the authors present two short ethnographic vignettes of how to frame medical issues within a larger context, drawing from their work with tuberculosis in Russia and HIV/AIDS in South Africa. The chapter explores some complexities, challenges, and advantages associated with being an anthropologist and a practitioner of clinical medicine, offering one response to the critiques presented by these students at Harvard Medical School. It highlights the need for full biosocial analysis linked to engagement in service delivery – the vitality of praxis – when tackling the most difficult global health problems today, drug-resistant tuberculosis and AIDS. There is, in anthropology, a literature on resistance: the resistance of the poor and otherwise oppressed to social and economic structures that shape the global burden of disease and disability. Biosocial analysis, drawing on fields from molecular epidemiology to anthropology, has the potential to advance efforts against tuberculosis and HIV.
