An increasing commitment to universal health coverage in the United States calls for examination of the policy problems involved in decisions to extend or to withhold health services to outsider groups such as illegal aliens. Congressional Hearings and other documents offer source material to identify areas of consensus and conflict involved in the exchange patterns between special interest groups and these aliens. Failures to achieve consensus in policies towards these aliens are associated with contrasting values and norms regarding the legal and the moral, the worthy and the unworthy in our society. The work of M. Mauss on reciprocity and gift giving contributes to an understanding of the relationship between economic value and altruistic behavior towards aliens. G. Simmel's analysis of the function of secrecy in social life underscores the need to undertake studies of hidden groups whose relations with society offer a broad vision of social reality
