Objective: To evaluate the trend in hospitalization rates due to diseases of the circulatory system in adults residing in the municipalities of the North Fluminense region of Rio de Janeiro, 2011-2021. Methods: Ecological study of time series of hospitalization rates for diseases of the circulatory system. Joinpoint regression models were used to estimate trends in hospitalization rates. Results: There were higher hospitalization rates in São Fidélis throughout the evaluated period. Conceição de Macabu showed a greater tendency to decline in rates (-65.18%; CI -81.94; -0.84) in the period 2011-2013. In the second period evaluated, only Macaé (2015-2021) showed a decreasing trend (-6.67%; CI -12.77; -3.14), while in Quissamã (2013-2021), there was an increasing and significant trend (11.54%; CI 5.54; 21.21). Conclusion: Most municipalities showed a decreasing trend in rates in the first evaluated period and in the second period, from 2013 onwards, only in Quissamã, the trend was increasing
