The paper has described the history of formation of the system of organization of medical care and treatment of surgical patients and victims on surface ships of the Navy in the conditions of distant voyages. A comparative analysis of the structure of surgical disease incidence of military men and operative activity on surface ships during distant voyages within the Soviet and Russian periods has been done. Special attention has been given to specificity of organization of medical care and treatment of patients and victims during anti-pirate activities of surface ships of the Navy of the Russian Federation in the Gulf of Aden. Ways of improvement of surgical treatment on ships of the Navy of the Russian Federation have been proposed
