The Efficiency Frontier of the German IQWiG in Comparison to the Classic Models of Markowitz and Koopmans The IQWiG (Institute for Quality and Efficiency in Health Care) argues that the use of its special efficiency frontier for the assessment of costs and benefits in the German statutory health care system is justified, inter alia , on the grounds that, more generally, the ‘efficiency frontiers‘ concept has gained prominence in economic research. In this context, the IQWiG refers to modern portfolio theory, which is based on Markowitz's famous article and, albeit indirectly, on Koopman's activity analysis. If these two ‘role models‘ are thoroughly analyzed, it becomes clear that they are not capable of providing guidance for upper price limits. Furthermore, the two concepts, despite some similarities, exhibit significant differences to the efficiency frontier employed by the IQWiG. Therefore, since this new approach cannot be justified by its alleged economic predecessors, its ability to assess possible price limits has to be proved on its own terms. An adaption of the model shows the necessity for an analysis of the willingness to pay for incremental units of utility
