One of the most important and pressing decisions we have to take about the future development of the hospital service concerns the size to which hospitals should be planned. Discussion of this topic began in 1864, when Holmes and Bristow1 concluded that : ' We have no hesitation in saying that the size of the hospital is a matter of absolute indifference; whether it contains a hundred patients or a thousand does not appear to exercise the least influence on the health of any individual in the hospital '. More recently, numerous ex cathedra statements have been made concerning the ideal size of a hospital, but as Lord Taylor2 has pointed out: 'The present conception of optimum hospital size . . . has been reached by rather imprecise socio-medical studies
