On July 30, 1965, President Lyndon Johnson signed the Medicare legislation into law. This legislation made no attempt to reorganize the nation's medical care delivery system that remained under private ownership and was composed primarily of solo fee-for-service physicians and independent nonprofit hospitals. Significant changes have occurred in the health sector during the past 20 years. In sheer numbers, the physician to population ratio has increased 41%, while the hospital beds' per capita ratio has risen 12%.1 Health care now accounts for nearly 11% of the gross national product (GNP)2 as opposed to less than 6% in 1965.1 Consequently, the goals of public policy have gradually shifted away from ensuring access to care for the nation's elderly and poor and toward controlling the rising cost of health care.
