A systems approach to understanding injury care in LMICs using causal loop diagrams
BACKGROUND: Around 40% of mortality after injuries is avoidable with access to quality care. Investments in interventions which maximise the number of injured persons who access and receive quality care are paramount, especially in low-resourced settings. Selecting such interventions within complex adaptive health systems is challenging due to the multiple interactions within these systems. METHODS: We created a conceptual map, or causal loop dia…
The intersection of emergency care, human resources and health equity
Strengthening systems of emergency healthcare is crucial to achieving global and national health goals. One of the key challenges in improving emergency care systems is the insufficient attention given to human resources for health (HRH) tasked with delivering this care. The intersection of emergency care policy and systems, human resources and health equity is an underdeveloped area of research and practice. In this project, we used HRH framewor…
Wicked problems and the recovery of meaning
Injuries cause 6 million deaths and 40 million disabilities annually. Over 90% of deaths occur in low-and-middle-income-countries (LMICs), and nearly half are preventable. This Commentary examines the possibilities and limitations of data to support injury care in LMIC settings. We frame injury care as a wicked problem with complex causality, contested goals, unintended consequences, and an incomplete evidence base. We use critical realism (CR) t…
Preventable trauma deaths and health system strengthening recommendations
One-third of deaths were preventable or potentially preventable, demonstrating a decrease from a review of 2021 trauma deaths in the same system which found 45% to be preventable. Findings highlight a continued need to increase training and strategic utilization of limited existing resources, such as CT and operating theatres, to improve the quality of trauma care. Preventable trauma death reviews are a powerful methodology to inform actionable c…
Defining and identifying the critical elements of operational readiness for public health emergency events
INTRODUCTION: COVID-19 showed that countries must strengthen their operational readiness (OPR) capabilities to respond to an imminent pandemic threat rapidly and proactively. We conducted a rapid scoping evidence review to understand the definition and critical elements of OPR against five core sub-systems of a new framework to strengthen the global architecture for Health Emergency Preparedness Response and Resilience (HEPR). METHODS: We searche…
Preventable trauma deaths in the Western Cape of South Africa
Injury causes 4.4 million deaths worldwide annually. 90% of all injury-related deaths occur in low-and-middle income countries. Findings from expert-led trauma death reviews can inform strategies to reduce trauma deaths. A cohort of trauma decedents was identified from an on-going study in the Western Cape Province of South Africa. For each case, demographics, injury characteristics, time and location of death and postmortem findings were collect…
