Objective: To understand care coordination for users with chronic health conditions from primary healthcare nurses’ perspective. Method: This is a qualitative descriptive interpretative study with the participation of 17 nurses who worked in Health Units in a capital city in southern Brazil. Data were collected from May to November 2023, through semi-structured interviews, and analyzed using an inductive analytical approach. Results: Care coordination is understood as a process of organizing healthcare, under the nurses’ responsibility, which is effective through the involvement of different people and services. Its potential includes care coordination, longitudinality promotion and contribution to improving users’ quality of life. Among the challenges identified are the high demand of people with chronic conditions, low adherence to treatment and fragility in human resources. The need to implement tools to understand users’ trajectory and improve communication between levels of care was highlighted as a way to overcome such obstacles. Conclusion: Nurses’ understanding of care coordination in primary healthcare demonstrates alignment with the principles that underpin this practice. This is an essential process to ensure comprehensive and longitudinal care, although it still faces challenges related to users, professionals and the organization of services, which require effective strategies to be overcome
