Intersectional experiences of non-communicable diseases and health seeking strategies in informal settlements in Freetown, Sierra Leone
This paper takes an intersectional approach to explore the burdens of non-communicable diseases (NCDs) and their interactions with gendered inequities and poverty, in shaping health seeking practices. The study explores the lived experiences of men and women living with NCDs in three informal settlements in Freetown, Sierra Leone. As Low- and Middle-Income Countries (LMICs) grapple with increased NCD risk factors, evidence gaps limit understandin…
Intersectional inequalities in healthcare utilisation in informal settlements in Freetown, Sierra Leone
Introduction Residents of informal settlements face significant intersectional inequalities, due to the overlapping and compounding effects of multiple social factors. This study aims to explore how these intersecting social factors, identified by community members, combine to shape household-level inequalities healthcare utilisation (HU) among residents of informal settlements in Freetown, Sierra Leone. The Economic Burden of Healthcare Utilization
Seven core competencies and conditions for equitable partnerships and power sharing in community-based participatory research
Equitable health research requires actively engaging communities in producing new knowledge to advocate for their health needs. Community-based participatory research (CBPR) relies on the coproduction of contextual and grounded knowledge between researchers, programme implementers and community partners with the aim of catalysing action for change. Improving coproduction competencies can support research quality and validity. Yet, frameworks and …
Ebola, informal settlements, and the role of place in infectious disease vulnerability
Studies of vulnerability often focus on the differential susceptibility of marginalised groups to the effects of disaster. This paper considers how vulnerability is also associated with the characteristics of place, especially the social setting of the informal settlement. In this light, it assesses specifically how cultural, historical, and political economic forces resulted in increased vulnerability to Ebola virus disease (EVD) within informal…
Sustaining Community-Based Responses to the Covid-19 Pandemic
In Freetown, Sierra Leone, despite the progress in the official response to the COVID-19 outbreak, there remain concerns about the limited involvement of local communities and the use of bottom-up participatory approaches in the response. While the official response has been characterised by restrictive public health measures such as partial lockdowns and the declaration of a state of a public health emergency, for the urban poor, the implication…
Exploring gender, health, and intersectionality in informal settlements in Freetown
This paper applies an intersectional lens to health in informal urban settlements in Freetown, Sierra Leone. We explored how intersecting social characteristics including gender, age, wealth, occupation, and tenant status influence health and well-being outcomes. We found that hazardous environmental conditions, poor waste disposal, and waste burning contribute to health problems at a neighbourhood level. Health-care access was also generally poo…
Covid-19 Response and Protracted Exclusion of Informal Settlement Residents in Freetown, Sierra Leone
Freetown has over 1 million residents, many of whom live in about 68 crowded informal settlements. Residents of these settlements struggle daily to access basic services such as water, sanitation, and health-care services. We found that the government’s Covid-19 response measures (curfews, lockdowns, and travel restrictions) excluded informal residents from contributing to its design, and the implementation of these measures prevented these resid…
How to prevent and address safeguarding concerns in global health research programmes
Safeguarding is rapidly rising up the international development agenda, yet literature on safeguarding in related research is limited. This paper shares processes and practice relating to safeguarding within an international research consortium (the ARISE hub, known as ARISE). ARISE aims to enhance accountability and improve the health and well-being of marginalised people living and working in informal urban spaces in low-income and middle-incom…
Local response in health emergencies
This paper highlights the major challenges and considerations for addressing COVID-19 in informal settlements. It discusses what is known about vulnerabilities and how to support local protective action. There is heightened concern about informal urban settlements because of the combination of population density and inadequate access to water and sanitation, which makes standard advice about social distancing and washing hands implausible. There …
Piloting a participatory, community-based health information system for strengthening community-based health services
BACKGROUND: Although community engagement has been promoted as a strategy for health systems strengthening, there is need for more evidence for effectiveness of this approach. We describe an operations research (OR) Study and assessment of one form of community engagement, the development and implementation of a participatory community-based health information system (PCBHIS), in slum communities in Freetown, Sierra Leone. METHODS: A child surviv…
