Screening and targeted sequencing of stool for microbiologic confirmation and drug resistance determination in paucibacillary tuberculosis
In 2023, an estimated 10.8 million people developed tuberculosis, and 1.25 million people died from this disease, including 161,000 deaths in people with HIV (PWH) in whom tuberculosis remains the leading cause of death. Detecting Mycobacterium tuberculosis drug resistance remains a challenge among patients with paucibacillary tuberculosis; since there is such low bacterial load in their sputum it’s unable to be detected via microscopy, there is …
Implementation of targeted next-generation sequencing for the diagnosis of drug-resistant tuberculosis in low-resource settings: A programmatic model, challenges, and initial outcomes
Targeted next-generation sequencing (tNGS) from clinical specimens has the potential to become a comprehensive tool for routine drug-resistance (DR) prediction of Mycobacterium tuberculosis complex strains (MTBC), the causative agent of tuberculosis (TB). However, TB mainly affects low- and middle-income countries, in which the implementation of new technologies have specific needs and challenges. We propose a model for programmatic implementatio…
Genomic epidemiology of Sars-CoV-2 during the first four waves in Mozambique
Mozambique reported the first case of coronavirus disease 2019 (COVID-19) in March 2020 and it has since spread to all provinces in the country. To investigate the introductions and spread of SARS-CoV-2 in Mozambique, 1 142 whole genome sequences sampled within Mozambique were phylogenetically analyzed against a globally representative set, reflecting the first 25 months of the epidemic. The epidemic in the country was marked by four waves of inf…
The effect of M. tuberculosis lineage on clinical phenotype
Six lineages of Mycobacterium tuberculosis sensu stricto (which excludes M . africanum) are described. Single-country or small observational data suggest differences in clinical phenotype between lineages. We present strain lineage and clinical phenotype data from 12,246 patients from 3 low-incidence and 5 high-incidence countries. We used multivariable logistic regression to explore the effect of lineage on site of disease and on cavities on che…
Collateral, Liquidity and Debt Sustainability
We study Markov‐perfect optimal fiscal policy in an economy with financial frictions and sovereign default in the form endogenously determined haircuts on outstanding debt. Government bonds facilitate tax smoothing but also provide collateral and liquidity services that mitigate financial frictions. A debt Laffer curve exists, which induces the government to issue bonds to a point where marginal debt has negative welfare effects. Debt positions i…
Pre-Columbian mycobacterial genomes reveal seals as a source of New World human tuberculosis
Insights Regarding Transdisciplinarity and Knowledge Transfer Gained from Two Case Studies on Integrated Water Resources Management in Ukraine and Mongolia
Transdisciplinary environmental research (TD research) seeks to solve problems arising at the point of interaction between ecological systems, the economy, and society. It seeks to enhance problem-solving capacity through interdisciplinarity and knowledge transfer between scientific and non-scientific actors. The article assesses how far the prerequisites for knowledge transfer are met in transdisciplinary projects on integrated water resources m…
The integration of two health systems: Social stratification, work and health in East and West Germany
Health Care Systems and the People: A Five-Nation Study in the European Union
The results of an exploratory five-nation survey of 2,239 adults interviewed on the telephone indicate general satisfaction with the health care system and less satisfaction with common welfare. In particular German respondents reported minor problems in their experiences with doctors with regard to communication and timing. Concerning health policy, French and Spanish respondents favoured more interventionist medicine, while Germans in particula…
