On the Power, Wisdom and Goodness of God as Manifested in the Adaptation of External Nature to the Moral and Intellectual Constitution of Man
The eight Bridgewater Treatises of the 1830s aimed to contribute to an understanding of the world as created by God. This, the first treatise, by the Scottish mathematician and churchman Thomas Chalmers, proposes an 'argument for the character of the Deity, as grounded on the laws and appearances of nature'. It sees harmonies between the intellectual and material worlds as manifesting the hand of God in their creation, anticipating aspects of tod…
A Survey of Current Problems in Meta-Analysis
The United States Agency for Health Care Policy and Research brought together representatives from each of the Patient Outcomes Research Teams (PORTs) as the Inter-PORT Meta-Analysis Work Group. The purpose of the meeting was to discuss a wide range of problems the panel had encountered in the area of meta-analysis. Several actual problems were presented, and tentative solutions to those problems were given. The PORTs had to consider issues such …
Economic Analysis in Randomized Control Trials
In medical technology assessment, randomized control trials (RCTs) play an important role in determining the relative efficacy of compared treatments. As scarce resources necessitate choosing among options for care, comparing costs of alternative tests, treatments, or programs also becomes important. This study assessed the prevalence and completeness of economic analyses in RCTs published from January 1966 through June 1988. It was found that on…
Peer Review Using a Paired-Comparison Technique
A paired-comparison technique is used to enable surgical residents and attending surgeons to make peer judgments of each other. Comparative peer judgments were made in three areas: the ability of the surgeon to make a diagnosis and to decide on a plan of active medical care, the operating ability of the surgeon, and the postoperative care of the patient. This method of peer judgment ensures the confidentiality of those making the judgments, and t…
Measuring the Quality of Medical Care
We outline the implementation of a new method of measuring the quality of medical care that counts cases of unnecessary disease and disability and unnecessary untimely deaths. First of all, conditions are listed in which the occurrence of a single case of disease or disability or a single untimely death would justify asking, "Why did it happen?" Secondly, we have selected conditions in which critical increases in rates of disease, disability, or …
