Equity in the ICU: An exploratory qualitative study of the ethical acceptability of a reserve system for allocating limited ICU resources
Dominant models of prioritising resources in intensive care unit (ICU) settings under conditions of severe resource scarcity, such as a pandemic, are likely to replicate and potentially exacerbate existing health inequities. One proposal is to advance equity by reserving a proportion of ICU resources, such as beds, for members of specific groups. This qualitative study explored the ethical acceptability of a reserve bed system (RBS) for healthcar…
Feasibility of software prompts to support antenatal vaccination decision-making: A cross-sectional study
Objectives: Vaccination in pregnancy is recommended to protect newborn infants against whooping cough, and pregnant people and their infants against influenza. Influenza vaccination is recommended at any time in pregnancy and pertussis vaccination is recommended after 16 weeks gestation. However, some pregnant people are not aware of recommendations or the importance of antenatal vaccination. This study explores the feasibility of adding a target…
Understanding police ‘reports of concern’ in child protection: Implications for decision-making, systems and data
If You Thought It Was Going to Make a Difference, You'd Do It Straight Away’: School Staff Decisions to Report to Child Protection
School staff make reports of concern to child protection agencies when they have concerns about child abuse and neglect. This decision has significant consequences for children, parents and communities, and for the data reports generate. Decisions occur within an ecological system context containing external, organisational, case and decision‐maker factors. This article reports on the findings of qualitative interviews with school staff in Aotear…
The heuristic divergence between community reporters and child protection agencies: Negotiating risk amidst shifting sands
Children enter the statutory part of ‘notify-investigate’ child protection systems via the reports of others, combined with acceptance by the statutory agency. This key nexus determines entry or deflection from statutory child protection services. To examine the decision reasoning and processes of community (non-governmental organisation) workers that underpin reports to statutory services. Participants are non-governmental organisation (NGO) wor…
He Pou Whirinaki (pillars of support). Elder indigenous New Zealand Māori narratives about influenza immunisation and the use of telehealth during the first Covid‐19 lockdown
This paper describes issues, strengths and challenges experienced by elderly Māori accessing influenza vaccination, during the first Level 4 COVID‐19 lockdown in 2020. We also obtained views about the introduction of COVID‐19 vaccines, and the social restrictions of lockdown. A sampling frame of four Māori elders speaking on behalf of themselves and their communities in Northland was recruited from health sector network connections and interviews…
The 2022 restructure of Aotearoa New Zealand's health system: Will it succeed in advancing equity where others have failed
Aotearoa New Zealand has restructured its health system with the objective of addressing inequitable access to health services and inequitable health outcomes, particularly those affecting the indigenous Māori population. In July 2022, two new organisations were created to centralise planning, funding and provision responsibilities for publicly funded health services in Aotearoa New Zealand. Health New Zealand and the Māori Health Authority have …
Examining spatial variation for immunisation coverage in pregnant women: A nationwide and geospatial retrospective cohort study in Aotearoa New Zealand
Data from our retrospective cohort study show significant regional variation in maternal immunisation coverage for both influenza and pertussis from 2013 to 2020. Maximal coverage was around 50% in the best performing regions, which means that half of the women who were pregnant (183,737 women) were not protected. In addition, we found significant spatio-temporal variation and clustering of immunisation coverage. Our findings are interactively av…
It's more personal if you can have that contact with a person”: Qualitative study of health information preferences of parents and caregivers of children with obesity in New Zealand
The objective of this study was to understand how participants referred to a childhood obesity intervention programme prefer to receive health information, and secondly, to determine acceptability of digital technologies such as a social media platform or IT application for programme engagement. This study includes a subset of interviews (n = 64) of a wider study of the barriers and facilitators of engagement in a multidisciplinary healthy lifest…
Locating transport sector responsibilities for the wellbeing of mobility-challenged people in Aotearoa New Zealand
wellbeing is a central tenet in the Aotearoa New Zealand government's Transport Outcomes Framework. Yet considerations of how place mediates diverse opportunities for wellbeing seldom identify how decision-making on transport and pedestrian infrastructure affects these opportunities. Considering the lived realities of older people and people with disabilities with a specific focus on Indigenous people, we argue it is particularly important to ide…
Challenges of making healthy lifestyle changes for families in Aotearoa/New Zealand
Objective: The objective of the current study was to identify challenges of making and sustaining healthy lifestyle changes for families with children/adolescents affected by obesity, who were referred to a multicomponent healthy lifestyle assessment and intervention programme in Aotearoa/New Zealand (NZ). Design: Secondary qualitative analysis of semi-structured interviews. Setting: Taranaki region of Aotearoa/NZ. Participants: Thirty-eight inte…
A Collaborative Indigenous-non-Indigenous Partnership Approach to Understanding Participant Experiences of a Community-Based Healthy Lifestyles Program
We describe the approach of an Indigenous-non-Indigenous research partnership in the context of a qualitative study which aimed to understand barriers and facilitators to engagement in a community-based healthy lifestyles program in Aotearoa/New Zealand. Informed by Kaupapa Māori research principles and by "Community-Up" research values, this collaborative approach between the mixed Māori-non-Māori research team effectively engaged with Māori and…
Indigenous voices on measuring and valuing health states
The philosophical assumptions that underpin the way in which health states are valued within economic measures of health are rarely made explicit and fail to capture the experiences of Indigenous peoples. Within a Kaupapa Māori theoretical paradigm, in-depth interviews were conducted with six Māori key informants who had cared for whānau (family) members through illness to give voice to dimensions of health and illness that Western economic measu…
Hitting the target without missing the point: New Zealand’s immunisation health target for two year olds
The use of health targets as a form of performance measurement has become more prominent internationally as governments have sought to control public expenditure, produce greater efficiency and improve accountability. However, health targets have faced criticism for their potential to produce negative outcomes within a health system. This paper examines how a health target was used as a policy instrument within the New Zealand health system to im…
