From fragments to overview – Challenges of gathering electronic health data to provide integrated care
Delivery of integrated care (IC) depends on accessing relevant patient information. The goal is for information to follow patient journeys, i.e., "one citizen - one health record". Today electronic solutions mirror the care silos they belong to. As a result, healthcare data is scattered across organizations and Electronic Health Record (EHR) systems. Thus, to create an overview of the patient journey across silos, EHR data from all relevant silos…

Digital presentation of the patient voice in pursuit of person centred, integrated and proactive care
Background: There is an overall agreement that it is important to shift focus from diagnosis-centric to person-centric care, where patients are active participants in healthcare services they receive. However, to support person-centric care for patients with complex long-term needs, the voice of the person must be present also in the electronic health record (EHR). However, the literature on how to design a digital "person voice" which captures "…
Learning from clinical supervision – a qualitative study of undergraduate medical students’ experiences

One way to improve psychological safety is to foster relationships between students and supervisors
Policies Make Coherent Care Pathways a Personal Responsibility for Clinicians: A Discourse Analysis of Policy Documents about Coordinators in Hospitals
We suggest that the policies' construction of the 'problem' as a responsibility issue, result in that neither diversity of patients' coordination needs, nor heterogeneity of hospital contexts regarding necessary system support for coordinators, is set on the agenda. Adoption of rhetoric from diagnosis-specific standardized pathways obscures unique challenges in creating coherent pathways for patients with complex needs
