Managing infectious mass casualty incidents in port environments strategy development and evaluation within the Armihn project
Background The ARMIHN project (“Adaptives Resilienz Management im Hafen”/“Adaptive Resilience Management in the Port”) aims to develop an operational strategy for managing mass casualty incidents involving infectious diseases (MCI-ID) in port environments. The strategy considers structural, organizational, and technical measures, accounting for end-user needs and local infrastructural characteristics, with the aim of optimizing patient care and f…
Participation behaviour of different migrant groups in breast cancer screening – palpation of the breast and mammography. Results from the German national cohort (Nako)
In contrast to earlier studies, our findings suggest that MSP participation and motivation does not significantly differ by migration status or language skills. This may indicate that information on MSP is broadly accessible through established invitation procedures in Germany. However, lower uptake of breast palpation by a physician in some migrant populations highlights potential gaps in broader preventive care engagement
Frailty among older adults in Germany
BACKGROUND: Frailty is a geriatric syndrome associated with increased morbidity, disability, and mortality. While its prevalence has been studied extensively in individual cohorts and patient populations, national prevalence data are scarce and regional variations within Germany remain largely unexplored. This study examined the frequency of frailty and its regional variation across the 18 NAKO (German National Cohort) study centers using a unifo…
Care delay during the Covid-19 pandemic in Germany – a cross-sectional online survey in the Nako study
During the COVID-19 pandemic, non-COVID-19 related healthcare utilization declined in Germany, resulting in care delay, including delays and cancellations of routine, chronic, and even acute care. The aim of this study was to investigate factors (i.e. regional differences and participant characteristics) associated with care delay during the pandemic in Germany using a cross-sectional survey. In October 2022, a total of 117,466 participants from …
Resilience of seafarers depending on occupational groups
Seafaring is a high-stress occupation that can significantly impact the mental health of crew members. Understanding the associations between resilience, stress perception, and depression is crucial for developing effective interventions to support their well-being. This study aims to investigate these relationships and explore potential differences between officers and ratings in resilience, stress perception, and depression. A total of 902 part…
Depression and anxiety symptoms in internally migrated women and men after the German unification
Subgroup analyses revealed group differences to depend on sex, but show negligible effect sizes, with internal migration only explaining less than 0.2 % of the variance for depression and anxiety symptoms. Internal migration alone is thus no substantial factor in explaining mental health differences. Future studies should use longitudinal data to determine temporal associations between internal migration and mental health
What can we learn from an intersectionality-informed description of study participants? Results from the German National Cohort
Our results showed that heterogeneity within social categories can be revealed by applying the concept of intersectionality when comparing study participants with an external population. This way, an intersectionality-informed approach contributes to describing social complexity among study participants more precisely. Furthermore, results can be used to reduce participation barriers in a more targeted way
Depressive symptoms, education, gender and history of migration - an intersectional analysis using data from the German National Cohort (Nako)
We observed a robust educational gradient in depressive symptoms, but gender and history of migration had substantial contribution on the magnitude of educational inequalities. An intersectional perspective on inequalities in depressive symptoms enhances current knowledge by showing that different social dimensions may intersect and contribute to inequalities in depressive symptoms. Future studies on inequalities in depression may greatly benefit…
From cigarettes to symptoms
Our findings support an association between smoking and depression. Robust dose-response relationships were found, with higher cigarette consumption associated with more severe depressive symptoms, and longer time since cessation linked to lower depression levels. These results highlight smoking as a meaningful and modifiable contributor to current and lifetime depression, suggesting that quitting smoking or reducing cigarette consumption may ben…
Childhood maltreatment and adult diseases in the general population
Childhood maltreatment is associated with an unhealthier lifestyle in adulthood and an increased risk of mental and somatic health problems, although the underlying pathways remain unclear. This study examined whether smoking and overweight mediate the association between childhood abuse/neglect and frequent adult diseases, including cancer, myocardial infarction, stroke, type 2 diabetes, chronic obstructive pulmonary disease, depression, and anx…
Occupational exposures and coronary heart disease in the Hamburg City Health Study (HCHS) – a cross-sectional study
This study is the first to assess the association of a wide range of occupations and occupational exposures with CHD in a German study population. We found no association between occupational exposures and CHD after adjustment, but 11 occupations associated with CHD were identified. The results are limited by cross-sectional design, healthy worker effect (HWE), and small group sizes. Further studies with a larger sample and longitudinal design co…
Mental health of individuals with pre-existing mental illnesses at the beginning of the Covid-19 pandemic
Background: The COVID-19 pandemic prompted a range of studies on mental health, with mixed results. Accidents, diseases and health complaints among seafarers on German-flagged container ships
The high proportion of health complaints and accidents among seafarers shows that there is a need to optimize medical care at sea and accident prevention, e.g. by standardized treatment algorithms or improving the medical training of Health Officers. The development and introduction of a digital patient file to record medical treatments on the vessels could also improve medical documentation on board
Exploration of Seafarers’ Mobile Proficiency as a Prerequisite for Possible Health App-based Health Promotion on Board
Numerous health hazards characterize the profession of a seafarer. The job-specific environment may affect the crews' well-being and mental health. The options for health promotion are limited, as seafarers are isolated for long periods and are a difficult-to-reach collective. A digital app-based health management system might offer a promising approach in this regard. This study aims to identify seafarers' technical competencies as a prerequisit…
