The paper examines the implications which the Federal-Provincial Fiscal Arrangements and Established Programs Financing Act, 1977, has for national health insurance in Canada. The Act not only reduces the amount of negotiation between the federal and provincial governments concerning health insurance 'standards' but also reduces the necessity for provincial governments bargaining with medical associations over the relation between fee schedules and extra-billing. It is plausible that this reduced bargaining, and consequently reduced public control, has contributed greatly to the development of user charges, which threaten the objectives and viability of the system
