Hospital wastewater contributes to the dissemination of Antibiotic Resistance (AR) when treatment plants are inadequate. This literature review investigates the prevalence of antibiotic resistance in hospital effluent discharges worldwide. A thorough search was conducted using databases such as PubMed, Scopus, Web of Science, and Google Scholar. The key terms included ‘antibiotic resistance’, ‘hospital wastewater’, ‘antibiotic-resistant genes’, and ‘environmental impact’. Data on ARM prevalence, antibiotic usage, and wastewater treatment practices were extracted and analysed. The results revealed high antibiotic-resistant markers in hospital wastewater across several countries, including those associated with multidrug-resistant (MDR) pathogens. Overuse of antibiotics in healthcare settings has been identified as a major cause of AR emergence. Inadequate hospital wastewater treatment increases the spread of ARMs, contaminating aquatic environments and endangering human and ecological health. Understanding this interplay is crucial for developing effective mitigation strategies
