Introduction: Psychiatric reform in Brazil aims to humanize treatment for mental health users. The Psychosocial Care Network (RAPS) came to replace asylums and offer humanized care. However, we observed setbacks due to the revolving door, in which users are repeatedly hospitalized and become chronic. Objective: We investigated the occurrence of the revolving door phenomenon in a mental health inpatient service of a university hospital in the Minas Gerais triangle. Method: Exploratory, qualitative and quantitative research analyzed the medical records of users with two or more hospitalizations in one year from 2018 to 2021. Results and discussion: We identified that the revolving door is recurrent. We characterized the socioeconomic profile of users, with the majority having 2 to 4 hospitalizations in one year, most hospitalizations were involuntary and compulsory, the prevalent diagnoses were schizophrenia and mood disorders, 56% were single, most had incomplete elementary education, young adults and men, 54% used alcohol and other drugs. The data point to the fragility of the RAPS and family support, the lack of preparation of professionals, and the tendency towards chronicity. Final considerations: The findings show the relevance of identifying the revolving door in the RAPS, which highlights the fragility of extra-hospital services in offering continuous care. The urgency of policies that strengthen deinstitutionalization and integration between health services and the community is highlighted, in order to end the asylum model and promote effective psychosocial reintegration of users
