With the increasing commercialization of clinical research and growing competition for research subjects, multinational pharmaceutical corporations are conducting more clinical research outside of primary market areas in the Global North. I refer to this increasingly international scope of privately-sponsored clinical research as ‘clinical globalization’, a term intended to encompass how politics, industry, groups, and individuals interact across national borders in producing knowledge and commodities related to health and disease. This paper explores the political economy driving research globalization, discusses the expansion of ethical and regulatory capacity that has accompanied international research, and uses a case study of privately-sponsored HIV/AIDS clinical research at a public hospital in Mexico City to examine some of the challenges faced by current ethical infrastructure
