HIV positive women, reproduction and sexuality in São Paulo, Brazil

Because of well-known differences in sexual networking and relationships, the AIDS epidemic in Brazil has been increasing faster in women than in men. The male:female ratio of HIV infection in São Paulo has risen from 29:1 in 1985 to 2:1 in 1998. We carried out a study at a gold-standard referral clinic for HiV/AIDS in São Paulo and examined the reproductive health needs of HIV positive women, factors that affected their access to HIV/AIDS treatments, safer sex practices and whether they could exercise informed choice when it came to having children. This paper describes what the 148 HIV positive women in the study had to say about sexuality and reproductive life. It also describes the process we used to present the study results to the women and their health care providers through support groups and educational sessions. HIV positive women need to have their reproductive and sexual rights respected, including in gold-standard clinics. Adherence to medication and adherence to safer sex practices are equally important and difficult and require both counselling and support. Counselling about reproductive choices for couples when one or both partners is HIV positive is an important form of secondary HIV/AIDS prevention. Services fostering a supportive environment to open up discussion about the sexual and reproductive rights of HIV positive people are urgently needed.RésuméAu Brésil, en raison de différences bien connues dans les réseaux et relations sexuels, l'epidémie de SIDA s'est répandue plus rapidement chez les femmes que chez les hommes. Dans l'un des meilleurs services de consultation VIH/SIDA de S∼ao Paulo, les auteurs ont examiné les besoins de santé génésique des femmes séropositives, les facteurs qui influent sur leur accés à un traitement anti-VIH, leur possibilité de protéger leurs rapports sexuels, et celle de choisir en connaissance de cause d'avoir ou non un enfant. Il décrit également le processus utilisé par les auteurs pour présenter les résultats de l'étude aux femmes et à ceux qui leur apportent des soins de santé, par le biais de groupes d'appui et de séances d'éducation. Les droits sexuels et génésiques des femmes séropositives doivent être respectés, même dans les services les plus renommés. Il est tout aussi important, et difficile, de suivre correctement son traitement médical que de protéger régulièrement ses rapports sexuels, et les patients dans les deux cas ont besoin d'appui et de conseils. Donner aux couples dont un partenaire, ou les deux, sont porteurs du VIH des conseils en matière de reproduction est une forme importante de prévention secondaire du VIH/SIDA. Il faudrait mettre en place d'urgence des services de soutien dont l'atmosphere favorise l'ouverture de discussions sur les droits sexuels et génésiques des séropositifs.ResumenLa epidemia del SIDA en el Brasil ha ido aumentando de forma más rápida entre las mujeres que entre los hombres debido a las ya conocida diferencias entre los contactos y relaciones sexuales. En una clinica de alta calidad especializada en el VIH/SIDA en Sao Paulo, examinamos las necesidades de las mujeres seropositivas, en cuanto a salud reproductiva se refiere, los factores que condicionaron su acceso al tratamiento del VIH/SIDA, a las prácticas de sexo seguro y si acaso pudieron ejercer su derecho a la maternidad manera informada. También describe el proceso que utilizamos para presentar los respetados del estudio a las mujeres y al personal médico, a través de grupos de apoyo y sesiones educativas. Las mujeres seropositivas necesitan que sus derechos sexuales y reproductivos sean respetados, incluyendo en las clinicas de alta calidad. Su adhesión a la medicación y a prácticas sexuales más seguras son de igual importancia y dificultad, y requieren tanto orientación como apoyo. La orientación en cuanto a decisiones sobre la reproducción para parejas, cuando uno/a o ambos son seropositivos tiene importancia en la prevención secundaria del VIH/SIDA. Hacen falta de forma urgente servicios que fomenten un clima de apoyo para iniciar el diálogo sobre los derechos sexuales y reproductivos de las personas seropositivas
